I didn't suffer with this issues until I was in my 40’s.  It would hit me in the middle of the night when I was sound asleep. Slowly coming on with the cramping becoming more severe. It has happened after orgasm as well. What I did come to find out is that stress has a lot to do with it. Subconsciously worries continue as we sleep even when we think we are fine. See if you are experiencing stress and worries and even guilt in your everyday life to see if it coincides. There are periods of time when I go without having these attacks - even years. It happened last night and currently in my life my Dad is going through terminal lung cancer and my life has been split in two as I try to manage two households. 

What works for me is having a muscle relaxer or Ativan (Lorazepam -.05mg) on hand. Meditation and doing pelvic floor strengthening and physical exercise. Work that stress out! 

Probably where the term “Pain in the ass” came from!
