Yes! If you know you are at risk for ectopic (being that you have had one) you should worry! If you are trying to concieve or having sex without protection, your doctor should have told you after the first ectopic how important it is to go in for blood work right away after 1 missed day of your period. Ectopics do not rise in HGC levels as fast as a healthy pregnancy so home pregnancy tests will not detect them very fast at all.
Brown spotting and/or pain are a big sign of ectopic pregnancy.
But... they are very hard to diagnose.

I had an ectopic 2 years ago. My doctor did blood work for a week to make sure and in that time I talked myself out of the pain I was in because I wanted a healthy baby so bad! I ened up in the emergency room. It had ruptured and I had to have my left tube removed. I later found out that woman die more often than known over this.

Bottom line - If you think or know you are at risk of an ectopic pregnancy get blood work done right away. The sooner you know the better!
