Hi, I have disabled spouse who has big needs and can barely mange to do all my duties. Is it matter of bad organisation or shuld I ask for professional help?

It is alsoimportant to remember that you have right to fullfil yourself as a whole personality. If there are also small children that you have to take care of, than there are additional duties for one spouse. It would be good if you can get help from close family, like grandmother, grandfather, sister, brother...
