Every few weeks for the past year or two I've woken up early in the morning (around 2-4 am) with severe testicle pain followed by diarrhea. The pain goes away within a few minutes but during it is unbearable. The only thing I can find related to this using google is this:

I ve been having the same problem as you . I consulted an urologist and after tests,scans,xrays, the doctor said everything appears normal and there s nothing to worry about. I was given antibiotics for a week.
But the pain goes off for a week and then the whole thing repeats.
Please let me know if you have identified your condition , would appreciate it.

I have had the same problem with intense pain and diarrhea. It has been going on for years, and was labeled Irritable Bowel. Now I find out I am hypothyroidism. You basically can tell if you are (the doc tests are limiting) Take your temp under your armpit as soon as you wake up and before you get out of bed every day for a week. If it is consistently lower than 98degrees you are suffering from hypothyroidism. It is a glandular issue that is way underdiagnosed because the blood test is faulty.. look around the internet at it and read the other symptoms and you will be amazed.

I have the exact problem. I always thought it was due to my lactose intolerance. Then someone suggested wheat intolerance and that fixed the problem. I started removing gluten from my diet and everything stoped from then onwards. I now can drink dairy products and yes was never lactose intolerant. Hope this helps and if not then it might be some other food type or spice.
