Ive been suffering with this for over 15 years. In the beginning it was thought to be Prostatitis, which we now know is not even a disease since studies have shown there is no evidence of inflammation in the prostate. So possible causes have come and gone. They were:
Resistant Bacterial Infection, Autoimmune disease, Pelvic floor dysfunction, Yeast, Nerve damage ( this is what I believe it is).
What I have done:
Multiple, multiple antibiotics, Gabapentin, Multiple multiple natural anti imflammatories, Hot baths, pelvic floor stretching ( That was fun), Prostate drainage ( Even more fun), Anti Anxiety medication, Multiple, multiple cord blocks, and finally nerve stripping surgery from Dr parrekattil in florida. I'm probably forgetting at least five things.
Nothing has ever even come close to touching the pain.
When I first got it it just came for no reason. I had it for five years and it went away on its own. It came back a few years later for two years and vanished again. It never really goes away, it is still very sensitive but not always painful. At the height of the pain I wore a hard protective cup. Airports were always stressful. It is back now. I check in every once in a while to see if there are any new treatments. Lots of quackery. I have seen at least ten urologists. Some have been great in working with me to try new things. Some are quacks that stick to the bacterial infection story when there is no evidence.
The only thing that has ever really helped is a strict course of ibuprofen. 800 miligrams every four hours will eventually knock it out for me.
Again, sorry to be a downer. I wish you all the best. Not everyone is the same and for some it probably has a real cause.

Guys.. This is my third attack of Epydidimitis in circa 10 years which started 5 weeks ago.. Everything is slowly going back to normal. I have taken 4 weeks of Cipro and I also spotted a video on YouTube which stated a great way to heal the epydidimis is the following. I have been doing this and it seems to be helping..
N-Acetyl-Cysteine (3 x 500mg per day)
Cranberry Extract tablet (3 x per day)
Epsom salt bath (4 x per week)
Probiotic (50billion cultures per day)
The above apparantly helps drain the Epydidimitis and aids healing.

What helps me with frequent urination - taking regularly high quality saw palmetto.
As far as tight testicle epididymitis - I noticed that since it is lower and backword hanging, it gets pinched between legs during a sleep. So positioning the testicle ( i.e. pulling it forward and locking in my underwear) seems relive the pressure during the sleep and makes it better.

I've noticed though like yourself that my testicles don't seem to be positioned in the same way as they used to. My pain is maybe a 4/10 absolute maximum (rarely). Usually, just tender and sits around a 1/10 and 2/10 but uncomfortable nonetheless. Plus I can't move much because it causes the pain to increase so I've been super careful.
I'm presuming that due to swelling it can make the normally lower testicle to sit even lower and go further back. But, due to the swelling it must be pushing the other healthy one into a "wrong" position, don't get me wrong it's not exactly horizontal but it's scary and I was scared to death of torsion! I've now had this issue around 3/4 weeks so I'm pretty sure it isn't torsion or I would've lost a ball - meaning I'd be in agony! Plus the ultrasounds said there was perfect blood flow there. But yeah, the left one is lower and further back (sitting diagonally) and my right one is "sitting" further up and forward (also diagonal). The urologist had a feel around (on 2 separate occasions) and said they were in a fine position. Regardless, I know my own set and they're not "sitting" right.
Sorry for the long post, it was just refreshing to see (and also sad) someone else who has been diagnosed with epididymitis where the positioning isn't exactly sitting right. I'm hoping once the swelling has subsided that they'll go back to their original position.
Anyway, have you managed to improve your condition at all? Any tips?

I never found antibiotics to be of any help really. Time and diet is the only thing that has helped. In the early days, it really freaked me out. I wore briefs to keep the boys in place, tried all kinds of different diets from wheat grass to fasting. Tried not ejaculating to ejaculating often. Over time, it's almost completely gone away although, I always somewhat feel a bit of discomfort but the significant pain part is gone.
I'm not a doctor by any means but they don't seem to know much about this at all. In my experience, having taken so many antibiotics I would say that the epididymitis ends up damaging the epididymis with scar tissue and making it chronic. Again, I'm not a doctor so I could be way off.
Anyway, my main advice for anyone new to this illness would be to stay calm and realize that life is not over. For me, the worst of it is long gone and just learning to live with it was the hardest part. Once you're past that, life goes on and a lot of the time, it's not a bother at all. And if you do not test positive for any bacterial infections to not over do it on the antibiotics. Doctors will be all too quick to prescribe them and get you out of their office.
This is my experience and hopefully it helps someone else get through these tough times. Cheers guys.
