Forehead Osteomas and Their Treatment

Forehead osteomas are a relatively common and benign condition where a knob of normal bone forms under the skin.  They can occur at birth, develop over many years or develop quickly.  There is not distinct cause although I have seen it be hereditary.

The bony knob can be single or multiple and can occur just about anywhere on the skull or face.  Forehead osteomas frequently occur between the hairline and the eyebrows.

These non-mobile lumps of bone are generally benign and cause not specific problems other than cosmetic concern.  It is interesting that some patients live with this condition for years because they don’t realize that the treatment is generally a simple office procedure.

Most benign osteomas involve only the outer portion of the skull and are not at risk for perforation deeper into the skull or brain.  I always perform a CT scan to make sure that there is no unusual plunging or deep extension of the lesion.

Figure 1 shows a typical forehead osteoma and the lower image shows a CT scan of the osteoma.

The treatment can frequently be performed with endoscopic technique through a button hole incision.  It can be performed with local anesthesia, but due to the instrumentation required to remove the lump and smooth the bone, IV sedation is preferable.  Forehead osteomas can also be treated with an open technique where the incision is hidden in the hairline.

Depending upon the size and location, the bony lump is reduced by using surgical instruments to trim it flat or grind it flat and smooth.  The procedure generally takes about 30 minutes and the recovery is minor.

Figure 2 shows a case of open treatment through a hairline incision.  The top picture shows the osteoma exposed, the middle image shows the osteoma being separated at its base and the bottom picture shows the lesion removed with a smooth base on the forehead bone.

Figure 3 shows the removed osteoma which is about the size of a small peach seed.

Figure 4 shows the same patient before and after osteoma removal from the forehead.

Figure 5 shows another case of before (top) and after (bottom) of a small osteoma removal performed with an endoscope.

I have a bump on the mid right of my forehead which has been there as long as I can remember. I am 28 now so its been about 20 years. Never grows or hurts, duno what it is but I did get bit by something on the forehead when I was around that age, which caused an allergic reaction that caused my face and forehead to look like a clingon or whatev from star trek. Pretty weird but doesn't seem to bother me.

I just got a tiny hard bone like bump dead center on the forehead. Noticed it after being very ill. Head pressure headaches all week. I joke to my husband I am turning into a unicorn and I hope he is alright with that. He is. Thanks everyone for your info and sharing. Sounds like mine is a cyst or bone growth. Hope it goes away. I have hit my head lots of times in sports and accidents. Am 40.

I have it too, for the last seven years. I've been to 5 doctors, including dermatologists and a plastic surgeon. I've had an MRI also. Nobody knows what it is, and the last doctor I saw referred me to a neurosurgeon. It's making me very nervous and sometimes it hurts. I've read there are risks involved with surgery, like nerve damage/facial paralysis.

I have a hard bump in the very middle of my forehead. The only think I can connect it to is when our 125 German Shepherd in a playful mood came running to me and i was is a sitting position. He knocked me over and I did see stars and think I passed out for a moment. No one was home so I went to the neighbors in case I passed out again. There was a huge welt in the same place the bony bump is now. That was at least seven years ago. The bump hasn't gotten bigger and it didn't appear immediately after the run-in with my dog. Several doctors have looked at it and aren't alarmed. I think it's a growth from the trauma to my forehead. I don't like the look of it and when people do ask me about it, I tell them it's my third eye.

I have a similar lump which I've had for about 20yrs and it's slowly getting bigger so I decided to have it surgical removed before it got too big, my doctor reffered me to a clinic and a doctor there tried to surgical remove it but failed and refferd me to the hospital, the surgeon at the hospital struggled to remove it also and just could not get it out, they 're not sure what it is so I am now waiting for the results of an x ray and blood test to see if that throws any light on the matter.
