You do really need a good instinct for it, and the ability to interpret what is going on in the partner's head and his body. It's a commitment, this is not just something you just decide to do on a whim.
A bit of commonsense doesn't go astray. Do not do this with someone if you don't trust his judgement. Good fisters are, by and large, good people, who understand other people and protect them on the journey.
Obviously preparation is needed. Right lubricants, poppers, right gear, slings etc, though I don't often use slings.

One, it is an anaesthetic, and it's going to stop the pain signals. Two, it dissociates you from the other partner (and what's going on around you generally.)
Taking something that blocks out pain receptors, dissociates you from others, and then getting fisted by what could be a total stranger, ignores the cardinal rules of fisting, that you need a connection with the partner and can read his mind and body, and that you need to be aware of pain, and what it is telling you.
As much as there's some pain tied up with the pleasure of fisting, you actually do want that pain. It tells both partners how they are going and if they need a break or to stop. It's there for a reason.
