Similar for me (57 yr old athletic build male).  Greenlight PVP on Feb 23, 2023.  Huge Foley Cath left in for 4 days, then flushed and removed at urologist office.  Stopped seeing blood / any particulate matter in urine after about 2.5 weeks.  Lemaon jiuce seemed to aleviate burning sensation.  However flow started to decrease in week 3 and then stop suddenly at start of week 4.  Did self catheter, but noticed no clots or blood.  Voided ~200-ml in catheter.  Went back on Flomax, but after 3-4 days still no improvement. Sitting on a firm chair has become quite painful (feels like sitting on a tennis ball).   With great straining I can naturally initiate a modest flow, but if I back off on stomach tightening it stops.  Initiating that flow is like doing some serious stomach crunches and if you back off on that stomach tightening the flow just stops.   Called my urologist when this first started 6 days ago (he is 7 hours away from small town where I live) and a few days ago he phoned to say it sounds like inflammation from possible infection.  I have no other noticeable infection signs, but who knows. They prescribed antibiotics and also a strong anti inflammatory, however after 3 days of those still no improvement...might even be getting worse.  Also tried Epsom salt baths, drinking 'Calm' (Magnesium Citrate drink), hot and cold packs.....nothing seems to be helping.   Presently I am self catheterizing once a day (5 days so far) in the morning, and then straining for up to a minute each discharge session the rest of the day to initiate urinating without the catheter.  Once I start urinating I can get about 50-75 ml out at a time.  Once the antibiotics run out in 3 days if still no improvement I'll go to local emergency to see if they can at least do a bladder flush just in case something bigger than can fit in my Coloplast Speed Cath (#12?) is causing a blockage. I hope that's the problem as the other options appear to be that either the Greenlight PVP surgery / subsequent big Foley catheter may have caused a 'Urethral Stricture'  which requires open surgery to repair and multi months recovery time (if successful...apparently ~80% chance on that), or perhaps having to do another TURP / cytoscopy procedure of some kind.   I'd be interested to hear if anyone else suddenly found they stopped urinating (without catheter or great stomach muscle straining) about a month after their GreenLight PVP procedure, and if so what ended up being the issue / solution.  Best wishes all.  John

It is not uncommon to experience blockage or difficulty urinating after a Greenlight PVP BPH surgery, especially during the first few weeks of recovery. This is because the body needs time to heal and adjust after the surgery.

However, if you are experiencing blockage four weeks after the surgery, it is important to consult with your urologist. Blockage could be caused by a variety of factors, including scar tissue formation or a bladder neck contracture, which may require additional treatment or intervention.

Your urologist may recommend further testing or imaging, such as a cystoscopy, to determine the cause of the blockage. They may also recommend medications or other therapies to help manage the symptoms.
