Bent and painful penis when erect. Black seed-like thing and white dry sperm-like under the foreskin

I have a downward bent penis for about 60 degrees. It gets really hard when its erect and somewhat hurts. On the top of that, I also have black seed like thing under the foreskin along with white sperm-like thing which smells very bad. Sometimes when the foreskin gets short, I can see all the black and white things under it and it starts itching. I also cannot masturbate due to the fact of bent hard penis. I am 27 and about to get married next year. I consulted an urologist and he advised me to go through a sonography and then do a biopsy to check what the black and white seed like things are under the foreskin. I am really worried about it, as it is time for me to get settled with a girl.
