I had Holep surgery with Dr. Bandi in 2015 and had a great experience. He was the fifth urologist I saw. Everyone in the community wanted to do a Greenlight laser. I had heard that Greenlight laser can cause a lot of irritative symptoms as the bladder gets some collateral damage and the reoperation rate is as high as 30%. I asked Dr. Bandi why no one would offer a TURP or Holep. He said that both procedures require overnight stay to observe for bleeding as a significant amount of prostate is removed. Most doctors in the community do not want to admit patients as they have to evaluate and discharge them next day, which their schedule doesnt permit. They would rather be in office seeing patients. In addition, some doctors make more facility fee if they do the procedure in a surgery center where they have an ownership interest. He also mentioned that HOLEP has a long learning curve (20cases) and requires more equipment, most surgeons give up during their learning curve. He learned holep from Dr. Lingeman himself. I am glad that I didnt have to go to Indianapolis and pay my 20% co-pay to get the procedure as that would have been thousands of dollars. I just paid my $100 deductible. I strongly recommend him for any prostate procedure (robotic prostatectomy or holep) in the Washington Dc area.

You do not need to go to the Mayo Clinic and although you urge caution with smaller institutions, it tends to be in smaller places the best people lurk. Dr Richard Batstone up in Bangor, Maine is an awesome Holep'er. He has done 1000's - some of the largest prostates in the US (pre-resection 500cc spreading into the bladder). Is an American who trained at Cambridge University in the UK, also has a PhD in Urology. Board Certified in 3 continents. His patients get almost no bleeding, home on the same day (no overnight in hospital unless they live alone or are particularly elderly), and back to normal in 2 weeks. Amazes me that someone like him is stuck up in the middle of nowhere.

Now that you have gone through the entire experience --- do you still feel that the extra preop two day visit was necessary? In other words was anything determined that made a significant difference? Reason I ask is that I also am in Florida and wonder if the two visits are really necessary. Thanks so much.

Attention Dr Lingeman patients:

Does anyone regret the fact that Dr Lingeman does not do a separate pre-HOLEP evaluation and work up???  It seems that many of the other HOLEP doctors require an additional pre-op visit and that often includes a cystoscopy. This is a significant consideration when choosing which doctor to go to.

Now that you have gone through the entire experience --- do you still feel that the extra preop visit is necessary??? In other words was anything determined that made a significant difference?

The reason I ask is that I live a long distance from most of the surgeons and I wonder if the two visits are really necessary.   Dr Lingeman (the "father" of HOLEP in the USA) does not require such a visit.

A question for recent patients of Dr Lingeman:

Dr Lingeman is on my short list of possible HoLEP surgeons.  In your opinion is Dr Lingeman still "at the top of his game"?????  I see no questionable comments in this forum, yet we all eventually suffer from the inevitable aging process.   Should I be concerned, considering that the good Dr is getting up there in years??

My HoLEP procedure was in May of 2016. Dr Lingeman was still very much on top of his game. My results were every bit as good, if not better, as my father's, who was among the early recipients of a HoLEP from Dr. Lingeman.

I too had to travel a long distance to get a HoLEP and neither saw nor experienced any issue with not having an pre-op examination that would have required two trips. Of course all medical records were sent to his office in advance. I would think that if there was anything that might be a cause for concern, he might ask for additional tests.

My only regret, is not having it done sooner.

I have HoLEP scheduled with Dr. Mitch Humphreys at Mayo Scottsdale for next month. I would appreciate any feedback that anyone can give me, positive or negative, about their experience with him. I have a 94 cc prostate, large median lobe and several hypoechoic regions, so my procedure time may take longer. The longer the procedure, the greater the risk for incontinence and dysuria due to increased manipulation of the urethra. I am concerned about both.
I would like to have only Dr. Humphreys' hands on me, but Mayo is a teaching hospital and I am concerned about annoying or angering him if I do not permit trainees to participate in the surgery -- any feedback??

I would like to recommend Dr. Bandi from Georgetown. I had a high Psa (32) and a small amount of cancer on biopsy. I was in retention. My urologist managed me with chronic foley for retention and active surveillance for prostate cancer. I was managed for about four years. I had multiple biopsies and multiple MRIs but he couldn't find more aggressive cancer. He kept thinking that the PSA is from the cancer. I got a second opinion after going through the reviews. My prostate was very large and Dr. Bandi performed a Holep. My PSA is down to two and I don't have to live with the catheter anymore. I am so thankful to my doctor for performing the procedure. I thank all of you as well for the recommendation.

The list above is pretty decent. I have called 3 doctors from the list and they had very similar profiles/responses. Everyone doesn't need to go to Indiana/Mayo to get a HOLEP. the learning curve is around 20-40 cases. Most people can easily find a competent urologist from the list above
