My urologist told me you can't do too many kegels but those are muscles down there like you said, and just like with weight-lifting, all muscles need rest/recovery time so I do 20 to 30 seconds on, then 20-30 seconds off, 5 reps a session, and do them maybe 4 sessions a day max to insure I do rest the muscles so they can recover. Just seems to make sense. Yes it is difficult being patient (not my strong suit) but what else can we do, right? Good luck!

Thanks for all of the helpful information. It's been a year since your post so I don't know if you'll see this.

My prostate is somewhere between 120-175 cc (different measurements, US vs. MRI). My PSA was from 11 but came down 2 points. I've had a negative biopsy and several negative MRIs.

I cannot sit, and I probably have chronic prostatitis from the biopsy - and my life hasn't been the same since the biopsy. I will not go through another one of those.

Sometimes during the day I can go 6 hours without peeing, and I void 250-400 cc. At night, it is much more difficult to go, and I am up between 2 and 5 times a night. I usually take 0.8 mg Flomax at night.

I've explored partial RP and talked with one of the top surgeons in the US. I also looked at PAE (Dr. Bagla, Virginia) and FLA (Dr. Karamanian, Houston). I like FLA the best, but I cannot afford it. PAE is also minimally invasive. I wish Dr. Bagla didn't rush me so much, as I could barely think of questions to ask him while on the phone. Dr. Karamanian is more giving of his time.

I realize HoLEP basically means RE, but it seems like it would give faster results than either PAE or FLA. I'm 60, and the thought of RE is disconcerting. But, truth is, I was 55 the last time my wife and I shared the bed. I'm also concerned about incontinence with HoLEP, which is virtually not an issue with PAE or FLA.

For HoLEP I am looking at Dr. Lingemann or Dr. Nicole Miller. I can drive to Nashville or Indianapolis. Getting to the east coast to see Dr. Bandi would be more difficult.

I thank God I haven't gone into AUR and have not needed a catheter, but I do think it's reaching the point where I must take action.

My Uro won't put me on Finasteride to shrink my prostate - which I thought would help make any procedure easier with less risk of complication.

I can not say enough good things about HoLEP by Dr Nicole Miller and her staff. The whole process and final results went amazingly well!!! And this is from a retired health professional who is skeptical of anything related to medicine.

I had Dr. Krambeck at IU Methodist in Indianapolis. She is an associate of Dr. Lingeman. She is a very talented doctor but my experience was that I felt that I was rushed through the initial consultation. In fact I has to submit no more that 5 questions in advance. She had some tests run then she answered my questions and ended the appointment. I really felt like I was on an assembly line. Following surgery I have had incontinence for about 6 months. This is what she predicted.

I have heard good things about Dr. Miller at Vanderbilt but I have no personal experience. The reports here are that Dr. Miller uses a modified technique that reduces or eliminates incontinence. I don’t know what the downside is.

From my personal experience with Dr Miller and her staff the entire process is very well organized, efficient, thorough, and Not rushed. Multiple levels of practitioners meet with you for discussion and question answering. I found that for all my major concerns I just asked the same question or expressed the same concern to each person that i spoke to, and sure enough it was taken care of very well. As a patient at Vanderbilt you are also provided with access to a "patient portal" where you can view your lab results, appointments, and have direct communication with the staff regarding your needs and concerns. Dr Miller also sets aside extra spaces in her schedule for out of town patients. Initially I had direct email communication with Dr Miller but I tried not to bother her directly after I gained access via the "portal". Just my opinion from experience --- and I have no connection with Vanderbilt or Dr Miller.

Thanks Phillip. If I had been in better health I would certainly considered Dr. Miller. She was on my route south. Her modification to standard HoLEP sure make the recovery very easy from what I read.

Glad to hear that you are doing better. It is amazing how our "plumbing" issues can affect our entire well-being. Yes, I believe Dr Miller's technique is a big improvement. It will be interesting to see how it works out in the long run.

Has anyone had HoLEP done with Dr. Joshua Leese at St. Joseph Mercy in Ypsilanty, MI? He reportedly performs a 100 HoLEP’s a year, but I cannot find any feedback on the results of his surgeries or any other surgeons in Michigan for that matter. I have HoLEP scheduled with Dr. Leese for the end of the year, but my prostate is significantly larger than an average patient’s, so if anyone can share their experience with Dr. Leese ans St. Joseph Mercy hospital, that would be extremely helpful. Thank you!

Another vote for Dr. Bandi at Georgetown. Fanstastic surgeon. The whole experience from the initial visit, prostate ultrasound to the procedure was amazing. Everyone in the hospital knew that he was the best prostate surgeon around. I shared a room with another patient who had HOLEP that day as well. OUR prostate volumes were more than 150 grams each. The irrigation was stopped next morning and the catheter was removed at 7.30 am and I was home by 10am. No incontinence. He mentioned that he routinely uses 2 modifications (en-bloc technique to decrease operative time) and antero-posterior dissection (to decrease incontinence). High recommend this doctor. Dr. Casey Andrew Dauw did my HoLEP procedure at the University of Michigan Hospital. He is an excellent surgeon and doctor. Very personable and highly skilled. My prostate was huge (230cc) very problematic. I was cathing 3 times a day per Dr. Dauw prior to surgery. He did many tests prior to my surgery day. I spent one night in the hospital and went home the next day;

Is the en-bloc technique the same as being used Turin Italy?

I hope you have less incontinence. I had it for about 8 months.
