I had my HoLEP surgery done a week ago. My Holep surgery was done at Vanderbilt in Nashville by Dr Miller. She is a great surgeron. I didn't have any pain after the surgery. The first couple of days after the surgery I had a little incontinence, but it is now gone. I has blood in the urine up to a couple of days ago, but is now gone. I am very happy with my outcome, I can now pee like a young person.

                I am a retired plastic surgeon and just underwent Holep at Georgetown with Dr. Bandi.  I live in upstate New York and was recommended a Robotic simple prostatectomy by my urologist.  I was told that I would be in the hospital for 1 to 2 days and will have a catheter for 5 to 7 days. I called around my Urology friends and they asked me to consider a Holep. My prostate size was measured to be 150 g on ultrasound. I was not in Urinary retention but had very bad symptoms and had two prior episodes of bladder stones. After reviewing the information here I got opinions in New York, Philadelphia and Washington DC. I could not  find a surgeon in an academic setting in New York who does Holep. I am a strong believer of getting complicated surgeries done at academic medical centers as they have the volume and expertise to support these surgeries. I saw Dr. Das at Jefferson who seemed like a nice guy. I ended up deciding with Dr. Bandi due to his excellent bedside manners, credentials, knowledge base and demeanor.  He answered all my questions patiently and was very upfront regarding complications including transient urinary incontinence and retrograde ejaculation. He seem to be regarded very high by his office staff, pre-and post operative room staff. I was discharged within 24 hours of the procedure without a catheter. The bleeding continued for about one week. I had minimal incontinence . I strongly recommend seeing Dr. Bandi if you are on the East Coast.

                I will be 6 months post-surgery on April 19 and am still having some incontinence issues and wear a Depends full-time.  We are warned about potential TUI (Transient Urinary Incontinence) in a small percentage of patients  -- it looks like I am one of the lucky ones!  I just hope that it truly is Transient.
Mayo Scottsdale has a Pelvic Floor Lab in each of its two locations as part of its PT Department. I went to the one on the Shea Blvd. campus a handful of times and basically was just told to do Kegels (which I have been doing daily). I have my first appointment at the Mayo Blvd campus later today to see if they tell me anything different.
Despite that, I am not letting it substantially negatively impact my life. I hike, go to the gym, work full time, etc -- and even just went on a two week vacation on a sailing yacht to Cuba (had to bring 2 weeks' worth of Depends, but I made it work!)

This is all very informational. I am a nuclear scientist, have a background in lasers/light energy and have done extensive research about the Holep procedure. My prostate measures 120cc and I have severe symptoms. I just realized that different surgeons use different lasers for Holep. The traditional laser is the Holmium laser. This laser is excellent for cutting and preserves tissue planes so the surgeon can differentiate between the adenoma and capsule, and remove all the adenoma. I saw some videos of doctors using the Protouch diode and Thulium lasers. These lasers coagulate the tissue better. There is less bleeding. However, the tissue planes were hard to see in the videos. It seems like the surgeons are creating arbitrary planes and leaving a lot of tissue behind. i am not a urologist but the videos easily show the difference. I will be getting consultations from a couple of surgeons recommended here but would recommend sticking to the original holmium laser. My 2cents!!

                I am 4 months post surgery and I too still have  incontinence issues. I wear CVS light shields and go thru 2 to 3 a day (usually 2 unless I am very active). For what it's worth, my local urologist, who is not the one who did my surgery, told me I will get it back, it just takes time and patience and TONS of kegels. He said do them as much as possible, anytime, anywhere, and that it is not possibls to do too many. So I am trying to stay positive and have faith that eventually it will resolve.                 Thanks Blayneb.  Yes, it is helpful to hear what you have been told.  I was told that you can do too many kegels such that the muscle would get fatigued leading to worse leakage.  I suppose everyone is different and we each have to try to find our balance.  I too was told to give it more time and. like you, I am trying to stay positive and have faith - which is sometimes challenging.

                I had the surgery 6 months ago, on October 19.   My understanding is that Dr. Humphreys is booking about 3 months out, but there often are cancellations that  might enable you to move your appointment up.

                Thanks for your reply and congrats on the surgery. I wish I would be there sooooonnnn!!! 3 months isn't a terribly long time for me, given how long I have been dealing with the condition all these years......Were you able to get scheduled during your first visit ? 
May I also ask what did they do for your first visit ? Were you able to get to see Dr. Humphreys for your first visit ? Apology if it feels like I am a dummy lol. I just really hope it could be the end of travail.

                I am very thankful to this forum.  I found my HOLEP surgeon here. 
I had a history of BPH for close to 20 years. I am 75 years old now. I underwent a greenlight procedure in 2010. At that time a urologist told me that my prostate was very large on examination. However, he assured that the high power greenlight can treat very large prostates. My symptoms continued after the procedure. If anything I was miserable for the first 3 months due to 2 horrible frequency and urgency to urinate. The doctor tried me on many medications. When the medications did not help, he did a cystoscopy and told me that my prostate was "open". When I continued to have symptoms, he performed a TURP in 2014. This time I had a catheter for 7 days and had a lot of bleeding even after the catheter was removed. Unfortunately, my symptoms did not improve.
I found Dr. Bandi on this forum and sought a second opinion. The first thing he recommended was to do a transrectal ultrasound to measure the prostate size. My prostate measured at 130 cc!! My procedure took 2 hours. The urine was clear next morning and my catheter was removed. He performed 3 other HOLEPs that date, which confirms that he is a high volume surgeon. I did have urinary incontinence which is getting better. I am only wearing a safety pad and can probably get by without a pad if needed. The doctor mentioned that the continence will likely continue to improve for up to a year.
For me the lesson learned was to get your prostate measured. My urologist never did a prostate ultrasound to measure the size. He went by prostate examination and cystoscopy. Dr. Bandi told me that the greenlight and the TURP only treated the tip of the iceberg. Hope this will help someone one day.
