Same here - back of the neck killing pain before and after ejaculation. Most unpleasant for a man - since insecure women could be inclined to think that they're somehow the reason of the pain, which obviously... is wrong.
So... it happens from time to time. As I read... it has to do something with the blood pressure, blood vessels and the heart (I have problems with first and the third for sure) or other serious health problems.
What I read few years ago is that some people have moments like this because of blood sugar level or something like that. So I tried eating one or two sweets after the sex, it seem to help. The pain was not entirely gone, but I was better. Well... if you try this do not exceed (after all sweets are good, but not in big numbers), just take a painkiller.

Ok maybe this could help, there is many different reasons to this.
For me, this pain was due to a contraction of a muscle. One day I gave my full determination to finish a round of push-up and when I got on the ground I had this horrible painful experience. (I remembered that I forgot to warm-up and eat something before my sport session, which is very dumb for swimming) since this incident, I was feeling this pain head when I was masturbating or doing trampoline.
We have so much muscles in our neck and the only solution that really helped me and would maybe help you too with your damaged muscles in your neck is to get something warm on it multiple days while being relaxed.
And I guess drinking water is great too.
I have been taking 2 pills of 200g of anti-inflammatories the morning, during lunch and my supper for 1 or 2 week.

I have EXACTLY the same problem and I was quite relived to see I'm not alone here.
I'm a 51 year old male, fit, healthy and active, living in the UK. I eat a healty diet and am well hydrated every day but I do drink a lot of tea and coffee, around 12-15 cups per day.
This pain started for me around six weeks ago, it came out of the blue, and it now occurs every time during sex or masturbation.
During/after the most recent occurrence last week I thought I was going to pass out because the pain was that bad. I couldn't move for about five minutes and it took several hours for the pain to fully subside. It feels like the blood vessels in my neck are being restricted or compressed, I can feel the blood pumping up in to my head.
I've been to the local doctors for a blood test, all normal, blood pressure was normal too.
Is it a long-COVID symptom? Or a side effect of a covid booster jab, or the flu jab?
Or have you all had some neck trauma? Is this musculoskeletal issue?
I'm looking for soemthing that we might all have in common as being the cause.
Did anyone ever get to the bottom of this? I'm quite worried.
