Mine's seem to fit your descriptions in every way except it's only a 2-5 times a year thing(not constantly recurring), probably closer to 2 times, and it recurs seemingly in the same spot. I do use alcohol or garlic powder(whichever I can get my hands on: prefer garlic powder), water, and soap to clean the entire area(head, too); maybe that has something to do with it or maybe not, but it's only about 2-5 times a year. In my defense, I use the antimicrobials on my entire penile area because I'm uncircumsized.
