I ended up having surgery within the six hour window to correct the issue. And recovered fully without any further complications.
A sports medicine doctor told me to start wearing a compression type undergarment like a jock for now on. The doctor asked me if there was any recording of my training at the track; there was. We used it for technical review. What they were looking at was the movement of my crotch area. It was a little embarrassing. It did show, while running the hartles, the lack of supported movement of the private area. I generally wore the track or split type running short at the time.

Ok so, I've practically been experiencing the same stuff. My testicle flips vertically 180 degrees. I know that because I have an epididymal cyst on it. So when I do feel my testicle, the cyst is on bottom not top. Thats how I know it's rotated by 180 degrees. I don't know whats causing this, but I'm quite freaked out. Should I go doctors? I've had it for around 4 days probably. Or forever, but I've only just realised / felt it within the past 4 days. Sometimes I have the slightest pain ever, but it's different to everybody elses cus they have a "painless" one whereas mine is a little bit painful. The pain carries onto my left groin. (very mild pain btw).

I had the same problems with Drs and ultrasound techs saying everything was Ok but it wasn't. You can use heat like hot showers and hot water bottles wrapped in a Tshirt to not burn your scrotum to overheat your testicles and try to get them to relax and drop back down loose and into their normal hanging position again. I've had a lot of torsions and the heat helps relax them and keep the blood flow open to save the testicles from harm. The less tight the twists in the spermatic cords, the longer a testicle can last before it dies. The drs do an orchiopexy surgery where they sew each testicle into the sides of the scrotum skin to prevent the testicles from twisting and climbing up and out of the scrotum permanently.
The ultrasound will only show blood flow and oxygen content in the scrotum and testicles. Its is the best way to see if any blood flow and oxygen are getting deep inside each testicle. But in less than obvious lack of blood flows the ultrasound will show the testicles are alive and oxygenating OK. But you can still have minor torsions that ache but don't stop the blood flow or kill the testicles. It just aches and hurts in the cords and has a dull ache deep inside each testicle as it happens. All you can do is try the heat and run to the emergency room if testis go numb or you have intense pain and get it checked for blood flow or do the orchiopexy surgery if you fear losing one or both testicles to this in the future.
But both my nuts got damaged and work less and lowered my testosterone levels and hurt my sex abilities and partly wrecked my manhood permanently from all this torsion damage.
