Sorry to say it, but you're going to be plagued with it until you're past 40, and will be in the 1% of men who have the worst case of it. Smegma, and the persistent smell of unwashed foreskin, is exacerbated by phimosis (the inability to retract foreskin past the lip/edge of the penis head)—and the more sensitive you are, the harder it is to clean, and the worse it gets. Only 1% of uncircumcised men who have reached full puberty have it. The area right around the bottom of the rim of the head (where the shaft meets the head) is where it accumulates the worst. Even if you COULD scrub that area thoroughly at least once daily, and then towel it dry aggressively (which are both needed tp smell fresh), your 'tightness,' and the lack of air getting in there, will bring the smell right back.
I love my foreskin, and love so many things about being uncut; but, if I had phimosis, I'd 100% be searching for surgical options. And, if it were the last resort, I would accept circumcision. Without fixing it, you will never be able to get a blowjob unless you're comfortable with sticking a cheesy-smelling penis all around the immediate vicinity of girls' noses. In addition, I couldn't imagine a life without free and easy foreskin movement, or without direct stimulation to the penis head. That's the best spot.
