I'd like to replace one of my testicles with one of my boyfriend's, and one of his testicles with one of mine. Has this been done before? What are the consequences? What effect could it have on my body? Would an ejaculation then contain both his sperm and mine? Where could such an operation be done? How long before you can have sex again after the operation? Are there any dangers?

There are several problems, the most severe being that either of you, or both of you, may not be compatible donors. You'd need to be tested, just as if you were going to donate a kidney. Even if you are compatible, you'll both need to be on anti-rejection drugs for the rest of your life.
Also, there could be issues with the "splicing" of the vas deferens. In a vasectomy it is cut and sealed and in a reversal they try to rejoin the ends. It does not always work. You'd have the same problem.
I don't know if your semen would be compatible with his sperm/vice versa.
I don't know where you could get this done.

To the best my knowledge, if successful, you'd ejaculate his and your DNA. But look at it this way, one sperm is either yours or his, it is not both combined.
