I don't think there is another reason, sometimes you wont have an orgasm because it's you doing all the work, me and my boyfriend done that today actually and it's brilliant when someone else does the work instead of you, i may be turning 13 in may but still, I have read many books and have experienced your situation;) I think that if you want an orgasm that bad try using a chunky pen(preferably one you don't use ever), never use a pencil, I done that mistake, ouchyyy :L
Try going in different poses too, and by the way, if I small bit of blood comes out and it hurts that doesn't mean you have started your period completely wear a pad just in case though, it just means you have ripped your hymen.
