I'm not a doctor, and my first piece of advice would be to see a GYN..
It sounds like there could be a multitude of reasons for the change. You may have a cyst on your uterus - causing the irregular and heavy bleeding. It may be just a change in your hormones. But, truly nothing to be so afraid you dont seek medical attention. You're so young, don't let good female reproductive health get ahead of you. Planned parenthood can assist you with low cost (sometimes cost free) birth control options. Look into a county health office if you don't have insurance to visit an Doctor. You're worth it!

I'd test now. I had a 21 day regular cycle too, and got pregnant 3 times. I always knew at about 4 weeks gestation, because I was so regular, WE have a wonderful 24 year old son. Unfortunately, I miscarried the other 2.
