I have had two embolization of the prostate artery procedures. They have failed to solve the problem. My NYU urologist strongly recommends that I undergo a TURP. He criticizes all other procedures. My reading of the research tells me that Holep is a better choice. Who and where in the NYC area performs Holep?

Never do TURP. HoLEP is the only way to go. You should be able to find a HoLEP doctor doing a search,

Hello: Go with Holep!!! Do NOT do TURP (that is stone age surgery).
On page 10 of this forum I posted a list of Holep doctors that I found shortly before I had Holep. That was at least 3 years ago so I know there are many more doctors doing the surgery now. There are two Holep forums on this web site. I suggest you read through both forums entirely and then you will know about all there is to know about Holep ..... and some other prostate surgeries. Specific doctors are mentioned throughout the forums as well as where they got their training.
Feel free to ask more questions on these two forums. They are a wonderful source of information from people who have gone through the experience.
You will not regret your choice if you go with Holep.

I am three months post HoLEP surgery. Everything is fine except I still have incontinence and have to wear a thin pad. I try to remember to do Kegels, but often forget. My bladder was stretched out a lot before the surgery due to urine retention (3 liters of urine was drained in ER two weeks before HoLEP), so I figure that may be why I still suffer leakage. I just wonder if anyone else had this issue extending past three months.

I am also in need of BPH surgery but I have seen too many folks in the ER and a couple of colleagues who have had horrific complications from “robotic surgery”. I have been reading about “simple prostatectomy” with suprapubic approach - I don’t know who is best with each procedure. Everyone claims to be the best and most experienced and I cannot find actual numbers of procedures done and complication rates. I have been septic twice from prostate infections and have not had more than 3 hours of uninterrupted sleep. I considered Green Laser, HOLEP, Rezum and simple prostatectomy- which I am sure IS a misnomer. Each urologist will recommend the one procedure they perform - getting an unbiased opinion is seemingly impossible.

If your prostate is large, HoLEP is the only procedure that will last. Others are for smaller sized prostates. Removing the prostate in total would be a great mistake. You would be permanently impotent. All nerve endings would be cut. HoLEP is best procedure because you will be able to urinate like a young man, have little to no pain, Maintain sexual function with good erections and climax with only dry ejaculation, only suffer incontininence until healing. Green Light is another lazer but not as good and can have major issues. Rezum may not last more than a few years. Urollift often does not work and no good for large prostates. So best is HoLEP.

I had holep surgery 12/5/20. Had mild incontinence for the first 2 months. After doing kegels every day the incontinence started to subside. Now at almost 5 months I only have a minor leak during incline set ups at the gym. I don’t wear shields anymore only during my set up routine. Again leakage is very light. I had my 3 month check up with my surgeon and he said everything was fine and to be patient. Sometimes it will take 6 months to completely go away.
I have a 6 month follow up in May. Hope to be completely dry by then.
It’s been a journey and looking forward to coming to an end.

Bob, you are doing better than I am. My procedure was about same days as you, first week of December 2020. I still have leakage. It's been 4 1/2 months. It so annoying. I was in a catheter for two weeks previous to surgery and had an extended bladder, so maybe that plays a part. I need to be more diligent on the Kegels. I still can't used to dry ejaculation. At least there is still a sense of a climax, though. I am 68. Blessings.

Lightfoot, It should go fine. You will wake up with no pain. Just have to put up with a catheter for overnight. Make sure you bring a large male Depends for the ride home. Your urethra will most likely be tender for a few weeks. Blood should stop within two weeks. You should be able to have sex after two weeks to three weeks. I waited 2 1/2 weeks, but it had been 5 1/2 weeks for me because I was in a catheter 2 weeks before surgery. If you happen to have a little leakage , it won't hurt your wife. Urine us sterile. It just takes time to heal. Your prostate took a lot of trauma, especially the urethra. Blessings.

Dr Lingeman of IU Health, From all I read during my research he is one of the top surgeons for this procedure. Surgery couldn’t have went better but I sure wasn’t prepared for the aftermath. I did not have any
retention issues before hand and have a relatively small prostate (85g).
Only time I had a catheter was about 12 hours after surgery.
I did really well for the first week. Had very little leakage and felt great.
Then I started down hill. By the end of week 2 leakage was getting worse. I would use a couple of the attends medium pads a day. Kept doing my kegels and things start to subside although I would have times when things would seem to get worse. I talked to the Dr. and was told this was normal and the incontinence was only temporary and would go away completely. I have my 6 month check up soon and am almost completely dry even during my set ups but still have that occasional squirt. If I prepare my self and tighten my pelvic floor muscle I have no problems. I am 72 years old and can pee over top of a car so I’m very happy about that but would definitely like to have even the occasional leak behind me. I wish they would have been more specific on the amount of kegels you should do though. Dr’s are all over the board on this one. My Dr was 5 sets of 5 for 5 seconds but I heard some Dr’s that had their patients doing up to 100 per day.
I know for a fact you can overdue these and your leakage will get worse. For all those considering this surgery, prepare yourself for the incontinence issue, I know the Dr will tell you it’s very rare, but it does happen as I can attest, but I would have the surgery again if I had to do it all over.
