I had HoLEP at IU in 2017. The surgeon, Dr. Krambeck, was technically very capable. I must say I felt that I was treated like a side beef in a butcher shop. Efficiency was emphasized. Since that time I have followed Dr. Krambeck’s twitter account where she is touting that IU patients no longer have to stay overnight.
Others on this group have mentioned Dr. Das in Philadelphia and Dr Miller at Vanderbilt.

I have registered w Vanderbilt n an apt w Dr Miller. I was told today a P of 220ml size is within her range for HoLEP. She has done much larger ones successfully. She only sees on a Tuesday as she operates all other days of the week. After initial consult visiting, the same day she does a Cysto to evaluate the P. Thereafter y r scheduled later for the HoLEP. Can anyone share their HoLEP. Check in, procedure, Catheter time, release, come back time? Pain, bleeding, clots, etc....

As I said I went the IU route. Before my initial consult I had to submit 10 questions in writing to Dr. K. I took my cat scan in, she looked at it and then came in and iintroduced herself to me. She had a nurse take urine sample talked to me a little about incontinence and then told me she had answered all my questions, told me to see her assistant about scheduling and walked out. A month latter I had my surgery.
Prior to surgery there were the normal limitations on my medications. The morning of my surgery Dr. K. sent a trainee to talk to me. I was rolled and the last thing I remember was being moved to the table and my feet being spread and being put into some stirrups. I stayed overnight with a rather large catheter in me. At about 4am it was removed and I urinated a couple of times and I was sent home around 11:00 am. I was fitted with a rather uncomfortable undergarment which the nurses apologized for. I recommend that you take your own paper underwear, don’t depend on the hospital!
For the first week or more it was tender to sit on a hard surface. Also you may have no control over you water. I had to change my underwear several times the first day. It took me 8 months to regain control. Even today I can lose control at rare moments. You may have an advantage with Dr. Miller. Dr. K. told me that she took 90% of my inner sphincter. At my follow up she mentioned that she is a personal friend of Dr. Miller. She also mentioned that Dr. Miller only takes about 65% of the inner sphincter. That difference could lead to a shorter period of incontinence.
I hope this helps. Let me know if you have more questions.

You should be careful with Thulep. Just watch some videos. Surgeons are carving out what they want as there is no tissue plane due to charring. Your reoperation rate may be high but bleeding is low. Stick to the gold standard: Holep

This is rather unacceptable. Looks like they just want volume. I want to see my surgeon in the office setting, ask questions and able to see them on preoperatove day. Looks like doctors at IU, Mayo and Nashville have no time to talk to their patients.
I have also heard that surgeons at some of these big institutions are running multiple operating rooms and letting the trainees do most of the operation. Make sure to ask the surgeon how many rooms they are running simultaneously. I know a neurosurgeon at Harvard got in trouble as the trainee did something while he was not in the room.

PAE only shrinks the prostate 20%. This can be achieved with finasteride over 6 months. It will never give the same outcome as holep which removes 2/3 of the prostate tissue. You will never know the outcome until you get the holep. I tried everything meds, microwave m, Urolift, pae for my 120g prostate. trust me, if your prostate is very large, just get a holep and be done for life...

I don’t look at the percent removed. I look at the ability to urinate without negative side effects.
BTW, my HoLEP reduced my prostate by less than 50%.
