Have not seen a doctor yet, but will. If I had to pick one thing, I would say it is something to do with the bladder. The sensation always has me going to the bathroom and urinating.
I am also someone that has always gone to the bathroom frequently (small bladder) - how about everyone else?

My own theory on the matter. The penis is an 'end of the line' organ so blood can typically stay here for a little bit longer than other places in the body. This is the same with your finger tips and toes. My only theory is, is that the alcohol travels to this area in the blood and ends up lingering for a while before it is moved on and around the system. I know i have a slightly faster than average resting heart beat and ever so slightly higher than normal blood preasure so this could explain why it happens to me and not others.. Please reply if you suffer from the same conditions. I know it isnt always too clever to draw your own conclusions to a matter but i feel mine is pretty harmless and will not effect peoples methods of trying to get this problem sussed and resolved.

I also have this problem. It only started last year when I was 29. Its whenever I drink any kind of alcohol and starts within 3 mins of consumption. The only thing that helps me is to take my time with my first drink. Take sips periodically for 30 - 60 mins and use something with a lower alcohol content. I have noticed wine works best for me currently. I haven't found a doctor that is familiar with this at all and continue to get strange looks whenever trying to describe the experience.

I thought maybe I had an STD so I went to a clinic and they thought I had one too. So they gave me a shot of antibiotic and did a complete battery of tests. All the tests turned out negative. Since then I have taken Zithromyacin "Zpack" for bronchitis. Neither of these treatments have affected my symptoms. The only thing that appears to help in my situation is if I take toilet paper and dry the inside of the end of my penis tip it seems to provide a bit of relief. Not sure why that is.
Several months ago I went to a urologist who is 64 years old and he checked my urine and did a physical exam. He was baffled by my symptoms. He said he had never heard of anything like it. He said my symptoms didn't make any sense for any of the conditions he is aware of. He said he would mention it to some of his colleagues and let me know if he found out anything. I haven't heard anything back from him.
Of course the obvious suggestion is just not to drink any more alcohol but that isn't possible! lol. Ironically when this started happening I hadn't been drinking very regularly. Several years ago I used to drink a lot more than I drink now. Currently I drink wine or beer, most often beer, about 1-3x per week and typically not more than a couple glasses of wine or 3-6 beers. However I have gone 2 weeks without drinking anything and this sensation will happen just like it always has. No better or worse.
Surely there is someone out there who knows what is going on here!!! I am dealing with it but it certainly is uncomfortable enough for me to want to seek a treatment.

To better understand the absorption of a drug and its distribution like ethanol(the alcohol we drink) we were given in school an example of a study in which a special chemical compound that causes a bitter taste in the mouth was injected into the butt of participants. From the time the subjects received the injection to the time they tasted it in their mouths was on the order of a few seconds or less. Fascinating =)

Just to say, I also have this problem and after a few sips of wine this evening, I had the very annoying onset of cystitus symptoms. I have read an article on the internet that says if you were prone to cystitus in the past, then it is very difficult to irradicate the bacteria that caused the problem in the first place... I am going to look up alternative therapies for cystitus, even though I don't have it, I think the damage has been done and there may be some natural way to treat it.
