During intercourse my body becomes cold and I feel dissatisfied, am I the reason we can't conceive?

Sir, I am 37 years old. I got married in 2012. But I have no child. When I was 13 years old, I practiced masturbation and continued till I was at 34. Due to bad practice in my childhood, now I suffer with many sexual disorder. During intercourse my body becomes cold and I feel dissatisfied during sex. Sometimes I feel back pain. Sometime the semen ejaculates with urine. I could not enjoy my married life & also I could not share my feeling to anybody who will help me to recover from this disease.
