Isn’t 20 years of experience enough for you? I reckon that your doc would notice if something was wrong and if you had varicocele-dilated veins!
Even if there is a swollen vein under your testicle, it is probably due to blood rush in them or just due to the fact that left testicles have more pressure put on them, then right testicles.
Varicocele usually appears above the left testicle because these veins and I suppose your doctor would feel it. They are felt when standing and they withdraw when a person’s lying down. When the patient bears down, the backflow can be felt.
You should have asked the doc what he thought this vein was when you were there!
Anyway, if you are unsure and feel like there’s something going on, you may ask for a second opinion.

So, let me get this right....

You're concerned that you can feel a vein or something under your left testicle, but when you have a doctor with 20 years experience that you're paying to examine you, you chicken out and don't tell him about your concerns. Is this right?

You need to have enough courage to tell the man what you want to know...

"Is this normal?" "Is this a problem?" "Will it get larger?" "Is this a varicocele?" "What can be done about it?" "SHOULD anything be done about it?"

As difficult as it may be for you to bring this up, it's better to ask a doctor who is right there and can HELP you, than asking an anonymous person over a blog or Q&A site.

Next time you're at the doctor's, ASK. If you're really concerned about this, schedule an appointment and TELL THE NURSE what you want to discuss with the doctor. That way it's on the agenda.

Good luck.
