Hello All,
I am a 39 yr old male with grade 3 hemorrhoids and some prolapse. Always bleeding and straining.
My Dr. has suggested the THD procedure but I must admit I am very scared. I have been to this site tons of times over the last year hoping to find peace of mind. I am still a bit scared.
I would like to hear from some of you who have posted here before. I want to know your thoughts now that time has passed. Did it reoccur, would you do again, how long did you hurt, did you have prolapse that was corrected? and anything else you can offer.

I've been dealing with hemmorhoids for probably 10 years now. a few years ago I started having a hemmi prolapse every BM. Recently it started to prolapse after about 30 minutes to a nhour of just walking around, and with 2 small children I just couldn't have it anymore. I went to Dr. Ramamoorthy at UCSD in San Diego, and first we tried the banding procedure. Relatively painless, but it didn't work. Then I brought her a picture of my prolapsed hemmi, and one look and she said I was a candidate for THD (the picture was very gross, but a huge help to the doctor). I was scheduled but was very nervous about this (or any procedure). Here's my advice: plan for your first BM by eating light the two days before the procedure, lots of soluble fiber, low on protein. I drank protein shakes to get my calories. I took stool softeners the day before my procedure. The procedure for me was not too bad. I woke from anesthesia with pain of about 4 or 5/10 (10 being unbearable). They gave me percocet for pain. The day after the procedure wasn't fun, pain in the 4-5 range for the night, percocets helped (though lots of sweating and chills from the drugs). No BM for the whole day, which was concerning. Day two I had a couple of small BMs (pencil width, an inch long). I continued on stool softeners, mostly ate smoothies, drank prune juice with benefiber, took milk of magnesia twice a day. Lots of stitz baths and regulars baths, as they are heaven and gave great relief. I would sit on the toilet for a minute or so and if nothing came out I just got up, and took another sitz bath. day 3 the levee broke, and I finally had a large (very soft thank god) BM. Now I'm only on ibuprofen, pain is very mild, and I'm starting to feel like myself again. I've found wet wipes with only water, and they are great for wiping. The whole thing wasn't that bad, and I'm grateful I did it. For the first time in years, I don't have to put back a prolapsed hemmi when I go to the bathroom. I"m only on day three, and I look forward to a full recovery. I definitely recommend the procedure, it seems to have worked wonderfully. The pain was not fun, but not unbearable. Even the worst of the pain was similar to the pain of walking around with a prolapsed hemmi in a place where I can't put it back. I know that it's very scary, but it's totally worth it. I think the folks who post with a lot of pain are outliers, and that my very tolerable experience is more normal. Good luck. You'll be way happier with your hemmi's behind you (pun intended)!!

1. Clavalin (an antibiotic) 1 hour before Surgery and for next 7 days
2.Proctol (hydrocortisone suppository) 1 hr before surgery and for 5 days
3. Fentanyl Patch (opioid) 1 hr before Surgery
4. Tylenol 1 gram 1 hr before surgery, every 6 hrs
5. Ketorolac (strong NSAID) 1 hr before surgery and then every 6 hrs
6. Oxyneo (another Opioid) 10 mg 3x/day
7. Pico Salex 1 at 2pm day before , 1 at 6pm day before
8. 2 Fleet Enemas 4 hours before Surgery

Doc suggested completely cleaning out bowels before surgery so no bowel movements for app 3 days after the procedure. This was good advice as the area is healing. The pain started app Day 3-4 with bowel movements. Able to contain with 1. Baths 2. Keterolac (strong anti inflam that doesnt cause constipation unlike opioids) 3. Proctol suppositories to keep swelling down.
Much better after 3 weeks but would suggest taking 4 weeks off work if possible to fully recover. A liquid diet is important for the first week and stool softeners for 4 weeks at least.
