I have been getting these pains to i am 22 i went to the doctor cause like all men i feared the worst. The doctor sent me for an ultra sound and nothing showed up on my testicles but when he checked my groin he found a hernia which he said would cause the pain when i ejaculate. Its to do with the path the testicles take when they drop which is left weakened and it is very common for men to get hernias in this place. I think the pains are related to nerve impingement in the lower back and sacrum areas (L1-L2 and L1-S). Sitting and sedentary days, middle body weight (gut) may contribute. If this is the case, then stretching the hip area, piriform muscles should/might help. I feel this pain on my left hip/thigh and it feels like someone has cut me. Its a horizontal downward at about 35 degrees, starting at hip, and ending on front of thigh, about elevation of crotch (where leg meets groin). What has been even more irritating is I am having some mild to moderate ED, and I am thinking this must be related as they coincided. I have not had either before 3 months ago. Another site says check Pudendal Nerve Entrapment and do piriformis stretches. I am going to try stretching and have the chirpractor stretch back. This is not good for a guys thats had rock solid virility (pun intended). But I am serious as to this. Will try and report back success or conclusions.

I am attempting to "reboot" my pelvis, and so haven't ejaculated in 26 days and don't know if I'll have the pain or not. I agree that it is nerve entrapment of some sort; however, after visiting numerous docts and chirocpractors, etc., over the last 30 years, I am taking matters in my own hand (so to speak) and experimenting with overall pelvic health: increasing adductor strength, loosening back muscles, strengthening the perineum (via kegels) and so on. I have even purchased an Aneros prostate massager and aside from the objective of overall toning the pelvic floor am also attempting a prostate orgasm as an alternative to the conventional way of doing things. All of this has precipitated my having to "make friends with my pelvis." There's so much to this. Overall, I am feeling better and am deadset on ridding my self of this ejaculatory pain. Seems that strengthening, awareness of the area, stretching, owning, and deep relaxation are key. Please post your thoughts as you move along wo resolve this issue. Good to know we are not alone, though frustrated at the lack of overall attention given to male chronic pelvic pain in the medical industry.

I have the same problem 20 years, but only when I am laying on my back. No problem in missionary position or doggystyle. Also no problem during oral sex when laying on my back. Maybe only few times. I had herniated disc problem, got rid of it mostly with exercises. I started recently very intense training program, that includes squats and core exercises and noticed, that these exercises kind of twist all my pelvic area, like it was in wrong position. So after few training sessions my ejaculation pain is almost gone, but now i have stomach pain, lower stomach area. And this happens in every position. So i guess this pain is related to lower back problems, some nerve is pinching or somethingduring orgasm. I remember when i started to feel pain in my left leg and lower back and was first diagnosed herniated disk, just before that i felt this ajaculation pain also during everyday activities.

I have exactly the same thing: pain in the front part of my left thigh. I am convinced it's related to back problems. In my opinion, it happens during the contraction of the inferior muscle of the pelvis, exactly during ejaculation. The muscle probably pushes and presses some small nerves, which painfully reflect at the front of the thigh.

I started getting this issue after I had a severe disc herniation that required surgery to remove the large disc fragment. I am lightly suggesting that you check to see if you may have a bulging disc that is pressing on your nerves when you tighten your pelvic muscles

How I know should not matter, but everyone who has this thing should seriously consider the possibility of chronic bacterial prostatitis.
Check with a urologist and try to get a SEMEN culture done.
