It is basically the result of nerve irritation while defecating.
The most important thing is to relax on the toilet. Don't force it too hard. Don't over-wipe when you're done. This can irritate the nerves and cause muscle spasms. The muscle spasms often lead to urinary problems.
Try adding more fiber to your diet and use vaseline if you are irritated.
Generally reducing stress will also help. Even simple stress/relaxation techniques like breathing or combing your hair can help if something causes pain to flare up again.

I have been having rectal pain, pain in my urethra, pain in the tip of my penis not related to voiding and while voiding urine, hell you name the pain, I've had it. The pain becomes more accute after sexual intercourse or masterbation. This has been happening for around 5 months now. I have tested negative for any std or infection on multiple occasions. I've had a three week course of Cipro, and a 10 day course of doxycycline. I have done hours of research on this, because to simply put it, I liked the idea of having a penis that functioned normally like it used to! I have become very frustrated with my Doctor, because all he can really do is supply me with new antibiotics with nasty side effects. This pain also more accute after smoking marijuana. Another person has mentioned the Headache in The Pelvis book on this forum. I have become familiar with their approach, and to me it seems like the only logical approach out there that I have been able to find at least. this is the url to their website and I strongly suggest looking at it. I have heard multiple success stories about this protocol and I am desperate to try it myself. However it is somewhat expensive I think ( in the range of 4000$). I will write back if and when I do undergo it. Until then I would say look at their website or keep doing whatever works for you because I've given up on the urologists.
I believe that the majority of the cases are caused by stress and anxiety and a poor diet, not a bacterial infection.
Figuring out techniques that relieve stress and anxiety such as yoga or pilates/ deep breathing techniques and hot baths are very important in helping deal with the pain of this problem. An unhealthy diet supplemented with caffeine and marijuana may irritate the prostate further.
I truly hope that one day their is a clear cut solution to this awful awful problem.

Yeah, I have the same problems!! I've had them for over a year now. Mine started with an initial "infection" that included more severe burning sensations than I have now and some clear discharge, but was diagnosed as Non-Gonochoccal Urethritis which seems to translate to "we don't know what it is." The Doxycycline seemed to clear that up, except for that little nagging occasional itch/stinging at the end of urethra - sometimes it feels like someone is sticking a needle into me right at the base of my penis head - and then yes, the anal itching and fullness developed after that - if I touch my anus, it feels like it's recoiling or something, like it's too tight.
Oh, and the worst part for me is that masturbation doesn't feel the same anymore - it's just not as enjoyable; there's a feeling like a sort of "stiffness" in my urethra, a slightly burning/hot sensation that's not very pleasurable.
Two different prostate exams resulted in them saying it felt a little swollen and "boggy." I think I've taken 3 rounds of Doxy, and an aggressive round of Cipro - which may have lasted a month, I can't remember now if it was Doxy or Cipro that they had me take for a month. 2 different urologists and a general family practice person - they even had me use scotch tape to test my anus for worms! they always seem to give up though - my last visit to the urologist, and he was kinda like "well it's probably not bacterial ... y'know, some people just have itchy anuses ... here's a prescription for what amounts to Hydrocortisone." What?! I lived about 25 years without this problem, so what happened? And gee, thanks for your topical band-aid solution!
I think this experience has shown me the limits of modern western medicine - all doctors can really do is prescribe antibiotics and steroids and perform surgeries, it seems like they're bad at little things like this, things that may require a more holistic approach.
So that's where I'm at right now, considering exploring other options - I got a book called "A Headache in the Pelvis" that attributes a lot of pelvic disorders to pelvic muscle tension. Unfortunately, I lack the discipline to really follow their advice thoroughly, especially because part of it involves finding someone who can do "trigger-point release therapy" which can even involve going in through the anus and pressing down on tense points - seems like it might be difficult to find a person trained in this. But this idea makes sense to me - why would my anus and the tip of my penis feel irritated? Maybe because they're connected to the same groups of muscles internally?
It also makes sense because when this first afflicted me, I was under a lot of stress, and it could be that my body was tensing muscles that I was not aware of.
Has anyone noticed any relationship with diet or anything like that? I feel like coffee might make mine feel a little worse, and possibly alcohol, but I'm not sure about that. I did this thing called the Master Cleanse last fall, a 10 day fast with only a maple-syrup-based-lemonade for sustenance, and absolutely no other chemicals, and I felt some improvements. In fact, I forgot about this, but before the cleanse I was even starting to get random pains creeping up my abdomen, and the cleanse (as well as exercise, yoga, etc.) seemed to abate that, but still the penis/anus pain/itching keeps coming back - it's actually getting worse for me right now, and I'm not sure why. I think I need to dive headlong into that book and see what happens.
I'm glad that I found this forum that describes my symptoms so accurately. I would be interested in working together to solve this. I'm tired of the psychological damage that this has caused, the feeling that I will never be well again, that I'm just going to have to live with this.

I must point out that the perianal itch isn't always a constant itch, but I do always feel as though there is some irritation there and that i need to frequently number 2 as a result of this. Masturbation/sexual encounters really irritate my meatus and i'm finding it difficult do frequently out of fear I will just irritate it more and more - bloody terrible to feel this way. I sometimes also experience a groin twitch/spasm, which i'd never experienced before these symptoms occurred. Since the stress levels have subsided so too has the frequency of these spasms.
At 3 weeks after the sexual encounter I underwent a STD check and all came back negative. I am seeing a skin specialist in the beginning of November to determine whether it is a skin problem and if not will undergo another STD check, as that will be 4 months after symptoms started occurring.
I have found that hot baths with salts really help relieve symptoms for a few hours at least and give you a sense of normalcy for a little while. This is indeed a very difficult situation to deal with and i'm very much at the point of trying something holistic if it is going to help me. Please post back anyone who has recently tried these methods to let us know how they went. I'm tired of living with this health issue, it is very degrading and really does effect your mental well-being in a terrible way.

I also have the same symptoms, burning skin in the genital region, and it is more pronounced when bored or sitting around. I am pretty sure my particular case is HPV. Unfortunately there is no medicine for getting rid of it aside from keeping healthy and letting your immune system fight it (taking vitamins, exercise, homeopathic remedies all can help just google them). HPV doesn't show up in male STD testing because there is no test for it (doctors will do anal swabs for gay men though). HPV is a virus of the skin, can be transferred even when using condoms via skin to skin contact, and many people who get it do not have visible warts. Normally HPV clears on its own in 1 - 3 years, high risk strains sometimes persist longer and in rare cases can develop into cancer.

No pain - more like hot feeling on tip of penis and hot/itchy anus and in between.
At times blotchy redness on penis tip (not always same place) - but inside of foreskin redness all the time.
Not an STD - but is definitely not a natural thing that I just woke up with all of a sudden (I can tell you the date this started), and not muscular or nerve related. It's not stress related either or my imagination.
Pretty much same symptoms as described by many here.
I reckon we notice it more when not doing anything because it's not severe enough to keep hold of our full attention when concentrating on something else but when we're not it just takes hold - and is irritating enough to make me miserable and sick of thinking about it.
One thing that nobody else has mentioned but is very apparent for me, is that water (i.e. bath or shower) gives some relief but causes penis tip and inner foreskin to get really red - it's like water causes it to react but only in terms of redness. Doesn't make it worse or better once dry. That's why I thought it was yeast or thrush but none of normal treatments/antibiotics for those are having any effect.
Maybe the water just causes the full extent of the skin irritation to show itself? i.e. maybe there is something there on the skin all the time that just isn't visible normally?
Canesten, which I've been trying for last few days seems to help with redness of penis tip and gives some relief but inner foreskin still redness so I don't think it's doing anything.
This HPV thing is intriguing - what's the treatment for that?
Ecoli too is another interesting one - again what treatment?
Somebody somewhere must know what it is and what we can do to get rid of it!!
