My 26 year old boyfriend has been suffering with severe pain in his penis and left testicle for the last 3 months. The tip of his penis/ urethra opening has a severe stinging and throbbing pain. The left side of the urethra opening also has a small bump with a dark purple discoloration (looks like an bulging small vein). The intense pain radiates from the tip of the penis down to his left testicle, then continues down his left leg and makes his big toe on the left side tingle and vibrate. He has been to dozen of doctors in Canada and the US, all of which have no idea what is going on. He has HPV which causes warts around his anus—could this be causing the pain? STDs tests have all come back negative for everything else. Under the impression that it could be a nerve problem, he has had 2 EMGs, an MRI—none of which showed any nerve damage. He has also had an ultrasound which showed a small epididymal cyst and hydrocele on the left side. However the doctor said that they are too small to cause any pain.
Along with the severe unbearable pain, he is unable to get an erection. If anyone has ANY idea as to what this could be please respond! We’re running out of hope.

It is hard to second guess the medical professionals especially with the diagnostic testing that has been done.

How long has he had the small bump on his penis? Did the pain start at the same time?

As for the pain, have they considered a kidney stone? It can cause groin pain, pain in the urethra, and pain running down the legs. Usually the epididymal cysts don't cause any significant discomfort and hydroceles are often painless.

Have they ruled out prostatitis, epididymitis, or any other infection? Are his testicles swollen?

Any lower back problems, maybe a pinched nerve?

I'd even consider shingles but with the length of time it doesn't make sense.

The pain would likely cause him difficulties in obtaining an erection.

thanks so much for your response! he did notice the small vein looking discoloration when the pain started;however, since the bump is in the opening of his urethra, its not in a location where he often sees, so its something that could have been there forever. could this be a blood clot? were thinking its more nerve-related because the way it travels down his leg. He has always has minor lower back pain because he injured it when he was younger. would have the EMG scans detected a pinched nerve?
His testicles are not swollen and they have ruled out kidney stones, as well as, hes been on several different rounds of antibiotics to rule out infection.

I 'm glad they've ruled out infection and kidney stones. It does sound like nerve related, especially with the long term lower back pain. It could be a form of sciatica.

I wouldn't even guess as to the bump in his urethra. It could be normal. I think it is unlikely that it is a blood clot as I would expect that he would have some discomfort.

An EMG is used to detect sciatica, MRI's may also be used. What were the results of the EMG test?

Both the MRI and EMG came back normal. The doctor did say something about the EMG would only detect nerve damage and not a pinched nerve/ compressed nerve? would the EMG for sure dectect sciatica? are there any other tests you could suggest that would detect nerve problems? He does have severe pain and discomfort at the tip where the bump is. anyway you think a vein and nerve could be related? thanks so much for your responses! i really appreciate it- and im sure my boyfriend does even more!

Hi,

Lets back up a bit.

The pain starts in his groin and runs down to the left foot and big toe. The sciatic nerve runs from the lower back and down the leg.

There is a condition called piriformis syndrome. The piriformis muscle, it runs from the lower spine to the femur, gets irritated and goes into spasm. It can be irritated by overuse, excessive sitting, falls, or other trauma. When this muscle goes into spasm it can compress the sciatic nerve and pain results.

A quick test, have your boyfriend walk with his left foot pointing outwards, since you said the left leg. You'd rotate the right foot out for the right leg. This takes the pressure off the piriformis muscle and it should reduce the pain a bit.

Your boyfriend may already have noticed that when he's trying to get comfortable that if he rotates the foot outward that the pain is reduced.

ENG should detect are commonly ordered for "pinched" nerves. In this case an MRI, focused upon the piriformis would probably be the best means of detecting any irritation.

I'm not sure about the bump on the penis. I tend to think the pain is related to the sciatica. Did the doctors look at it?

hi im 20 and have some of them syptoms too i get back pain and leg pain on the left side and the toe thing to i have noticed like vains or blood colts on both of my legs i dont no what they are but they are a funny couler also i have a lump near my left testie but not on it just really close to it i get alot of pain from that too its been there since i was 17 but never botherd with it before but now its really starting to bug me .

Hi,

I have been battling this problem for a year now. I'll tell you what the problem is. Its a nerve compression in the spine between L5 and S1. Dont worry, quite often MRIs miss to detect it. This problem causes pain in the perineal region and down the leg. Usually it is one sided but it coudl also affect both. Your bf needs to start reinforning his lower back and correct his posture. Tell him to start swimming, twice a week, only front crawl. And he should also start a series of exercises to strengthen his lower back. Chack out sciatica exercises in google. Take it is first. It will get better with time.

Thanks for sharing your story, its comforting to know he's not the only one who is dealing with this! how did you figure out a nerve compression is the problem- any tests you can suggest? its been over 8 months now and the pain continues to worsen- he's on intense pain medication which does help to take the edge off a little. is it something that he will have to deal with forever? do you also have a small bulging vein? he seems to think that if they remove the vein the pain will stop but i think thats just wishful thinking. is there anything else you can suggest? thanks for your message

I have the same issue but also with a sharp pain in my inner anus canal, I'm currently being treated for Prostate infection due to it being a little enlarged I have been on cipro for a month and had a psa test however go back to the dr. in two weeks to take another psa to see if it changes. Good news so far no more pain in the deeper part in the anus however the pain in left leg and groin area are still there dull but still there. I'm hoping no prostate cancer, you know how us men jump to the worst however with the way chemicals are in food, air , water....who knows. I wish you the best keep trying different doctors, you may want to consider accupuncture???

I have had the same symptoms as well and come to learn that it is the pudendal neuralgia.

The pudendal nerve stems from the S2(lowerback) and has different branches that lead to the scrotum, anus, tip of penus, etc. There are very few doctors in the world that deal with this nerve so therfor help is hard to find. PNE is pudendal nerve entrapment and happens when the nerve is entrapped in one of the spinal/pelvis canals it runs through and PN (Pudendal neuralgia) happens when the nerve is irratated by another problem. Doctors suspect that mine was caused by a tear in my l4-l5 disc that leaked chemotoxins down onto my s1 s2 irratating the pudendal nerve. As for my leg and feet pain it's because of the pudendal nerve cross talking to other close by nerves such as the sciatic nerve and etc.

Has he tried using a penis health creme?  These cremes are great for all sorts of skin issues down there.  It might help with some of his external issues for sure.  But only a doctor would know about the internal pain.  Try a creme though. They are really helpful.  Good luck.

It sounds to me like it's the prudendal nerve! Do your homework. I'm almost ready to guarantee you I'm right on this issue. A friend of mine had the same problem and he also went to see a dozen doctors. No one could seem to figure out what was wrong with him until he came across a wonderful urologist out of New York who completely fixed his problem. His issue ran from the ankle up through the knee to his scrotum to his prudendal nerve. It takes a specialist to find the problem and when it's found there's a number of ways to fix the problem, but it also depends on how bad things are damaged. The sad part is if it's not fixed ASAP or fixed by an expect in the prudendal nerve field you can be sh*t out of luck. And your man might not ever be able to get wood again without an implant. So please do your due diligence and find the right specialist in the prudendal nerve field and handle your business. Good luck to you....

I have this exact same pain, if you can let me know how your bf is would be great, because I'm paranoid as hell about it, lol. but yeah, even so I can look for solutions. Or anyone else with any ideas, as its the penis, to the left testicle and just under it and going down to my left foot which is really bizarre to think about.... some one if possible please drop a message/reply when you can.
