I would guess you are meaning a pimple on your sack and are not feeling a lump on your testicle inside.
So yes it is just a pimple you can get them on your scrotum sometimes that is why it is important to practice good hygiene wash your penis and scrotum everyday and if you’re not circumcised make sure you pull the skin back as far as you can and clean the head of your penis as well.
Your groin area can get very warm and perspiration happens down there as well and pee can drip on your bits as well so keep everything good and clean.
