After the first time, it may have been a niggling worry in the back of your mind that made it hard for you to keep an erection. If there are other stressful things in your life then that could be having an effect on you without you even realising it. If I were you I would explain to your ex girlfriend that it is not something you can control and that her having a go at you made the situation worse. Go to the doctor and tell them as much information about the problem as you can ( can you sustain an erection alone, had you been drinking) and he/she will be able to help you with the problem.
