I also get this red rawness, soreness in my butt crack from tailbone to just above anus opening. When it's bad, I also have that rawness in the crease between my thighs and genital area. Oh so painful. The first time, my doctor tested for STDs (none) and diagnosed a bacterial infection. Prescribed Nystatin and Triamcinolone Acetonide cream. It took a while to heal because the area doesn't get much of a chance to completely dry out. It comes and goes for me every now and then...maybe twice a year. Anyway my doctor suggested I use diaper rash ointment (the zinc oxide type rather than the petroleum jelly based) as a protectant if I work out or go swimming and won't be able to get out of those wet, tight clothes immediately. She seems to think a cause of this infection is the area allowed to stay moist for a period of time. I haven't been faithful to this suggestion which may be why the infection returns.

Never use any oil based antispetics on the wounds nor vaseline, as this can cause a huge infection wich will be leaving one unable to sit down comfortably for days. Before the open wound occurs use baby powders to keep it dry down there especially in summer when one tends to sweat more, if you pad your legs/butt crack with baby powder a couple of times a day and ensure everything is dry the skin will be unlikely to tear and no open wounds. When the skin tears and there is an open wound I am afraid that powders wont work anymore and can now only cause an infection. What you will have to do then is clean the wound with a water soluble antispetic (like savlon - diluted as per instructions on bottle) then dabb the wound with mercurochrome or gentian violet and leave till dry. When merc. or gen. is dry put a few layers of gauze over the wound and try to keep it as dry as possible. When you wipe your butt use either baby wipes or spread a thin layer of aqueous cream over the toilet paper before wiping as this eliminates the direct roughness of the toilet papers fibres against your skin which can be abrasive to people with a predisposition to these tears and infections. Keep on using the aqueous cream wiping method forever as to prevent the toilet paper abarading the skin leaving small open partches of skin which allow bacteria and yeasts to enter. If you have a bad infection you will unfortunately have to go to the dr. for antibiotic and steroid creams as then the above steps will not help you at all, BUT THE BEST WAY TO PREVENT THIS FROM HAPPENING IS TO USE POWDER TO KEEP THINGS DRY DOWN THERE BEFORE THE SKIN ACTUALLY TEARS OPEN FROM BACTERIA/YEASTS.

I've been getting an itchy red line, about an inch long in the top part of my crack. It recurs. It usually starts out itchy then bleeds. I have tried everything, I always need to wait till it goes away. Never asked a Dr about it. I hope it's not a pilonidial cyst, those are a nightmare. My dad had one removed, I'm female but quite hairy. I think it seems to occur less frequently if I wear a cotton thong instead of other things that can trap moisture.
