Hello---I have all of the above mentioned symptoms. I am thinking of the pudental nerve. I researched and many symptoms match. I have had pelvic floor physical therapy, it has helped . I would suggest asking your doctor about pt.

I've been experiencing this same exact pain since August 2012. I've been treated with 4 different kinds of anti-biotics and had samples of urine sent off to be tested. They came back clear. I've had pain in my kidneys, so the doctors have been treating it as though it were a UTI, however the extreme and prolonged doses of the antibiotics they have given me, haven't shifted it. I don't believe I have an STI/STD as I lost my virginity last year, to my boyfriend who only a few months beforehand had been checked for everything and was not sexually active again until we got together. It's my next port of call though. What I would say is that peeing after sex does help lessen the pain and how frequently an attack of it occurs, and i've noticed that if you work to relax your lower stomach/bladder area it does virtually stop the pain. If you think about it, that area tends to be the first part of the body that tenses up when you're stressed, so maybe it's linked to that?
These are just my ideass/experiences of this. It's nice to know i'm not the only baffled one out there! Good luck to everyone!
