My wife and I started having anal sex 14 years ago. I was also concerned about the possible health issues involved. I spoke to a Urologist and a Proctologist. Both said it was safe, as long as there was very little pain involved. According to the proctologist, if there is very much pain, that means tearing could be taking place, which can damage the sphincter muscles or, if the pain is internal, you could be causing a fissure (a tearing of the rectal tissue). These are both bad.
Surprisingly, the Urologist said I (as the giver) was just as likely to get an infection, such as a urinary tract or reproductive tract infection, from oral sex as anal.
Both of these assume you, like my wife and I, are a monogamous couple who is free of diseases. Otherwise, anal sex is a great way to spread sexually transmitted disease.
My wife and I have been having anal sex about five times a week now for 14 years, and haven't had any problems as a result.
