I had this same exact issue to the point I couldn't touch it for days on end and it kept splitting open. I eventually discovered that a mixture of coconut oil and tea tree oil would allow it to heal quite rapidly. I occasionally get flair ups but if I use it fast enough, and be gentle with my extra curricular activities, it heals up again within a day or two.
Ingredients:
2 table spoons of coconut oil
1/2 a tea spoon of tea tree oil (too much is counter productive)
Mix them together in a seal-able container and place in a cool dry location when not using. Wash hands thoroughly after use.
Instructions and Warnings for use:
Wash and dry your penis thoroughly with warm water and soap (preferably anti-bacterial)
Apply at your discretion to the affected area and always wash it again before reapplying.
Be extra gentle if you can't resist masturbating and wash your penis thoroughly before having sex as tea tree oil is for topical use only and does not belong inside a vagina (can cause irritation and can be poisonous if too much is absorbed through the vaginal walls)

I’m using Herbicide antiseptic soap this thoroughly cleans the would out, then I pad it dry then apply bag balm to the open wound I sleep naked to get get plenty of air flow so it naturally dries out, so far after three days seems to be working I’ve stopped wacking it, I’m diabetic so wounds are too heal this works for me good luck guys your not alone.

Hey guys, I'm going to second Protopic (tacrolimus .1%) from a doctor.
I had the exact same friction issue after exercising in tight cotton boxer briefs. It wouldn't heal for months and I tried everything: Vaseline, coconut oil, and shea butter (only very temporary relief), prescription-strength hydrocortisone (made it worse), tea tree oil (made it a LOT worse), and just leaving it alone to air out. Nothing worked, it hurt all the time and ruined my quality of life.
A few days of Protopic and I'm feeling way, way better. I'll post an update if there's any relapse, but so far it seems to be making a huge difference and the skin barrier is healing. The doctor visit is awkward, but getting help is definitely better than chronic pain down there!

I only came back to this post to help you guys out. I had what I thought was a scar on my penis from a friction burn that seemed to get worst each month. Turns out it was a rash created from the scar not properly healing. Went to the doctor finally not am sad clinic and she prescribed an ointment called Tacrolimus. Bro...I got my life back that's all I can say.

I'm having a similar issue caused by applying rubbing alcohol on my penis. The area has been painful and sensitive ever since. I've spent weeks trying aquaphor, vaseline, over the counter healing balms, etc. I tried steroid creams but I'm pretty sure they were making it worse because I started feeling this sensation even when nothing was touching the area. My derm recently prescribed me protopic. So for anyone that used this, did you apply anything else in addition to the protopic or just used protopic and nothing else? The issue hasn't resolved but isn't getting worse. Just not sure if I'm properly moisturizing or not.
