I'm 52 year old male. Since few days I start to feel heart pain only when I'm coughing. I'm smoker. It is like pressure on my heart and it is only focused excatly in the brest.

Well you are 52 years old and you have been smoking, probably, most of your life. I am sure that you are aware of the bad consequences of the smoking on your heart and especially on your lungs and on your heart.
So, the first thing is first, if you don’t want to stop smoking you need to see your doctor and do certain tests to find out why you are feeling this pain. Since you are coughing it could be lungs but you are feeling chest pain so it could be your heart and you will not know until you do analysis and tests.
After that you will know what you should do to prevent the pain from coming back.
