my penis is 4inches long and soft well here goes the second proplem I can stop thinking about sex (I am imbaresed to say this) and I feel to have the need to have sex..is that unusual ???thats why my penis is almost always hard.

If your 4 inches erect for 13 that is just fine if it is that soft your well above average.
The always thinking of sex is normal lots of hormones influencing your thought waiting until your older is well worth doing self pleasure will take care of the urge.
Frequent erections are part of puberty as well and very difficult to control when and where it happens. It sounds like you are developing just the way you should be and experiencing the same things most guys do during puberty.
