My 13 year old son has been having pain in his rectum. We cannot find out what it is. He complains of severe pain in that area. what can it be?

I suppose you have already seen a number of doctors to determine if there were any signs of hemorrhoids, anal fissures, etc..although he seems to be too young for any of these.
There could be two possible causes that his doctors may have not remembered. One of them is Levator ani syndrome that appears to be present more in women that in men but it does occur in men as well. This kind of syndrome causes spasms of the muscles that appear around the anus and that can be quite painful. The pain may last up to twenty minutes and the cause of the spasms is not known. What you could try is soaking in warm tub and massaging the muscles. Painkillers could be taken to get rid of the pain just make sure you get those that are safe for children.
Proctalgia fugax of unknown origin and characterized by painful session i.e clusters. The pain may be so severe; it usually appears at the opening unlike the previous pain that is felt deep in the rectum. It may appear every few weeks or months.
