I have been suffering from this for around 15 years or so. I managed like you guys and it was just an annoyance and cause for fear I was going to hurt my penis somehow it would get so hard. I impregnated my wife three times in that period.
I will say this to you guys, my life became a living hell after getting a vasectomy because of this condition. DO NOT, get a vasectomy with this condition. The combination long lasting erections plus the scar tissue can cause excruciating pain. SRPE was an annoyance, add post vasectomy pain due to issues with scar tissue and long intense night time erections and it was like pure torutre. I got my vasectomy reversed, which cleaned out a lot of the scar tissue, and the pain, subsided quite a bit but I still have issues sitting and with pain that are pretty horrific.
This condition does not spontaneously go away. I am in year 16 or so.
One ray of hope is that I've noticed it gets much better if I am not working and sitting in a chair 8 hours a day. I believe mine is aggravated made worse by chair sitting.
Again, because of my vasectomy, this condition causes me a lot of pain. Do NOT get a vasectomy.

I haven't seen a doctor about this (SRPE) yet (definitely going to get on that), so it could be something else (not sickle cell though). I have seen a doctor about sleep before and had a high respiratory disturbance index on a home sleep study so I am need to look into tests sleep disordered breathing some more (haven't yet because of recent move).
Just recently I had the idea to try sleeping in an arm chair through the night. I don't think I am used to sleeping more upright yet (from what I've read it is something you can get used to), so my sleep still hasn't been good; but as far as the unusually strong and long lasting erections, they seem gone. I still get erections in the night of course, but they are much more normal, not of the same sort or duration.
If I had to take a wild guess at where this problem comes from I would look at brain blood flow.
Has anyone else tried sleeping sitting up?

Long story short, I was also using for a number of years Rogaine foam - which contains minoxidil, for hair loss. This prevents hair loss by maximizing blood flow in the scalp. After years of use, I have discontinued the product for a week, and after a couple of days the erections went back to normal.
So my advice, check what (if any) medicine you are taking, ask your doctor if it increases blood flow and if possible discontinue for several days to see if it improves your sleep.
Also another piece o information, I have had a bad experience with baclofen: it was helpful but it creates a strong addiction and severe withdrawal symptoms.

This John answering the question about the size of the wedge pillow I'm using. I use a 15 inch wedge pillow which can be found on amazon for about $30.00. I don't sleep all the way inclined but about halfway up the pillow so a lower incline may work for some as well. It seems to make it "better" but I still get up about 2x a night on average. A new interesting correlation I'm finding is drinking carbonated beverages. As I increase my carbonated beverage intake, I get up more and have more intestinal gas. Something may be connected there as well. I had digestive changes around the same time this started as an FYI.

I had noticed that if I am sitting on my knees with my body over my legs (essentially a child's pose in yoga) that my erections would go away pretty quickly. It isn't comfortable for me to sleep this way though because my legs fall asleep, but I realized that all is needed is for one of my legs to be pulled up to my chest and my erections will be normal (meaning normal NPTs). So, I now sleep on my side with the lower leg pulled up to my chest.

Guys this is a big deal. I always thought there was a link to Propecia. There have been class action lawsuits against Merck of Post Finasteride syndrome. I believe this could be good fodder for a lawsuit. This condition has really wrecked my quality of life. It started right exactly when I went on Propecia. The poster named Eugene has surveys he has collected from many guys here. This board is kind of dumb. It's completely anonymous so you can't even check to see who has and has not taken propecia on this thread based on their handle.
