My husband has a red spot on his big toe, between the toenail and the joint in the toe. For lack of a better descriptive term, looks like a hickey. It is not painful for him. He says he has had this for a couple of months now, but was not concerned about it until last night. Now he noticed two very small spots, similar in color, on other toes. He is very concerned that these could be kaposi sarcoma.

Unfortunately there's really not so sure for me to tell over the internet whether this is Kaposi's sarcoma (KS) or not. There are many other lesions which look like red spot on toes, which could be other than KS. Basically any lesion, as well as red spots on toes, which does not heal should be examined by biopsy. This is the only real way to know what this red spot on toe means. KS is clearly much more common and happens to HIV infected individuals. There is an interesting question now, could this be a non-HIV related KS, if that is KS at all? Also HIV negative KS tends to be much less aggressive than the HIV positive. You need concrete answers, so the blood test for HIV will help. He may eventually need a punch biopsy of one of his skin lesions and red spots on toes. This is a relatively minor procedure easy to perform, so that is the best way to discover what those spots are caused by.
