It would be no more unhealthy wearing knickers with tights than not wearing knickers with tights, if anything is would say it's probably more healthy not wearing knickers as the skin would be able to breath better just in tights.

Hi, can I join as a guy? Being one I can't know from experience, I know some, probably most prefer with panties, some don't, there's nothing malicious in that, just a preference, I only come to think that it might be better to have breathable, non synthetic tissue, like the cotton of panties at contact with intimate parts. Some tights, though, have cotton tissue build in in that zone, to ease going without panties.
Some even wear panties over them, apparently to hold them up, but still I think most also wear them under.
