Thank god for this f*****g thread. Thank you for everyone who has posted, it has pulled me out of the deepest malaise of my life. Hopefully later in this post I will have a valuable contribution.
I have these symptons: sitting down aggravates, stringing red scrotum, stinging thighs, irritated groin, constant moving around to get comfortable, cant wait to get home after work to warm shower and lay down, tight jeans aggravate considerably, planes are pretty much a nightmare.
At the age of 40 I was going bonkers persistently seeking 'skin' solutions to no avail. Gradual decline over the past for years has worn me down despite: many dermos and urologists (some of them downright dissmissive and rude), UVB therapy, patch testing, biopsies, countless creams/lotions/gels (anti-fungals, anti-septic/baterials, corticosteriods etc), elimination dieting, repeated blood testing including STDs and Lupus markers, psuedo-allergy diet, Ultrasounds, Urine testing, anti-histamines, tens of thousands of dollars in various trial error supplementation, traditional chinese medicine and more! I have missed out of the parasite killing herbs so far bit haven't ruled it out. Have done a couple courses of antibiotcs but am yet to play the 'Doxycyline fad' which seems to have some Japanese research backing it up for some skin problems.
Some of them worked for a while, some of them were miraculous for a while, they all eventually failed. B comp + B2 and B12 worked for a month, Chinese medicine worked for 6 months (I thought I was saved), Tinactin worked for three weeks, Candida diet rarely worked but always reduced symptoms a little. Large protein hits such as steak, denver omletted, etc, still improve things about 50% strike rate and last for a few hours to half a day (which is why I have added immunologist to my future battle plan)
PNE or CPPS seems the moslty likely given that what I have researched and read here. I am pushing this agenda now with a new doc who is supportive and understands this scene. I have built a large spreadsheet with all possible combination of pathology with explanations, markers/symptoms to pave my way through any ignorant medicos along the way. As we all know the average GP will only think 'give this guy steroid cream and get him out of here so I can get to the next patient'. Each wants to get paid not be the next 'House' diagnostician.
Next steps for me:
1. get referred to a PT that can get me started on trigger point management (CPPS), then self-manage with one of those custom designed wands
2. use a NASID or Nerve medications if it helps - test it out
3. add to the mix natural nerve pain remedies - ALA has strong trial support
4. Get a new diagnosis from a Nuero who knows this field well (CPPS, PNE, Meralgia Paresthetica, pelvic myoneuropathy), consider my routes after than: natural, drugs or surgery of indicated
5. See an immunologist (very rare to find one who is actually trained) who specialises in this for a different angle, I have a name of one now
6. Take up Yoga and stretching for CPPS
Anyways, thats my plan, I feel better now, even though I loathe to go out with people now if it involves sitting down.

I just found another forum (still no solution) with a lot of people with the same things. it's on medhelp and is titled: Penile pain, burning and itching genitals. The first post describes it really good i think. (except the penis pain, which i myself don't have).
We are definitely not alone, in those posts you can also read what other people have already tried and tested.
Currently my symptoms are much much better, i also sometimes do not think about it anymore because i can't feel it. I don't know why? I did take probiotics for a month if that could have anything to do with it or it's just the cold (freezing) weather over here, which does lessen the symptoms like i mentioned before. And also sat for a long time on a U or O shaped cusion which did help just like there are no symptoms when sitting on a toilet seat, but later it did not help anymore.
If the temperature also effect you or if your in a pretty warm room try to open a window or make the area your in cold to see if it has any effect. I sometimes can not sleep because of the burning, then i open my window a little to make it colder and that helps. I just wake up having it a little cold...I also tried using a ventilator to cool up my room that also helped.
someone on the other forum did mention something about vitamin D deficiency and after taking vitamin D it went away. I also use vitamin D because i am deficient also. A lot of people have low levels of vitamin D. I don't know if this has anything to do with what we have.

In case this has not cleared up for some of you. On most of the cases it seems that your problem is jock itch. This does not only belong to jocks or fat people it is just a build up of fungus that likes moist warm regions, unfortunately thats our groin area where the thigh meets. Easiest way to get rid of this is with a form of jock itch ointment however if the area is too moist this will not always work so you will have to use some form of compress to dry the area out especially if you have what may be a rash looking blister like bumps. If it goes out onto your penis then it is probably not jock itch and more likely that it is an STD. To the people having a tingly sensation then you are looking at poor circulation (can happen in perfectly healthy skinny people). So, i hope that helped a little. If the creams and compress do not help then you have a more serious issue probably dealing with a bigger infection.

HK, I have the exact symptoms, burning tingling red scrotum and burning in entire groin area for over a year now, I have tested negative for all std's blood and urine tests. I have been back and forth on whether this is nerves or bacteria, but im almost certain its a combination, but even in saying that i am still at a loss. Mine started after protected sex, and it could have resulted from an aggravation from a previous back injury i had already had. If you have any headway in your symptoms, or diagnosis please let me know. I am desperately searching for a resolution.

I think I can top you all...I have been suffering from CPPS for, gulp , 22 years now! Two divorces, and now single alone for the last 16 years. I have tried everything mentioned. I seem to have some relief from Evening Primrose Oil tablets but just when I thought it was working, it came back. It can burn for 3-6 months, take a few months off, then come back again. I do believe cold weather does not help. I am currently seeing a acupuncturist for the past month and no change yet, but I am encouraged by what I read...especially with electroacupuncture, which I have not used yet.
I am actually sitting on a hot pad as I write as it is soothing, if nothing else. I have found that sleeping with your legs out rather than bent helps. No doubt sitting is horrible and I only wear loose pants or athletic pants. The burning goes from the tip of the penis to the rear of the buttocks...varying in areas along the way. Urination feels good temporarily but is worse a few minutes afterwards. Arousal and ejaculation make things worse. It certainly has changed my quality of life, chronic fatigue has set in as well as depression at times.
I have read many articles for many years...guys, they do not know what causes this so they do not know how to treat it. I am curious to see if any of you live in a year round warm climate? I will let you know if the acupuncture works.

Could it possibly be caused by one of our jeans? a specific brand or store. Not sure but i might have happened after (not immediately) two new (similar) light colored jeans. But this is just a thought i'm not sure.
Another thing is Stress. Currently i don't even think about this condition, because i almost don't have it anymore. But sometimes i definitely feel it. Thats when i am sitting with jeans on, when i feel a slightly uncomfortable in my mind because i think of something else that i want/should do instead of what i currently do which i don't like than it seems to be getting started again. So it could be kinda caused or aggregated by stress or something similar.

In most of these cases posted here it is indicated that you may be suffering from a form of Peripheral Neuropathy. This *may* be nerve damage from hyperglycemia (high blood sugar) or the beginning of Diabetes Milletus. In these situations this condition can cause a yeast infection in the pelvic region which has escalated subcutaneously to the nerve ending level (worst case maybe necrotizing fasciitis). If it is not topically treated by good washroom hygiene and some topical mitigation powder like a menthol and zinc oxide combination (i.e. Gold Bond) it can become much worse. It some cases the odor is simply appalling. In any case you must deal with the skin that is being infected.
The Gold Bond powder will help restore the skin damage. The proper hygiene will control the pelvic infection (note: this includes your genitalia too). You must wash-up EVERYDAY if not several times a day as the infection can get out of hand quickly. Especially when you sweat down there. A little soap and hot water with through drying and then some Gold Bond will leave you smelling of Gold Bond but within a few minutes you will notice the stinging/burning sensation subside. Next for hyperglycemics... control your diet. Eat more fruits and vegetables. Stay away from complex carbohydrates like sugar. Get a Diabetics cookbook and stick to it. No more candy and pastries. Ever notice the awful odor developing a few hours after after gorging on sweets? And the frequent urination and thirst? And men have you noticed the erectile dysfunction yet?
If your situation is not because of Diabetes-related condition above, then I do not know what your problem is. It certainly sounds like some sort of nerve damage. If a Urologists can't help you seek out a Neurologist. If you are not against Wholistic Medicine seek them out too.

I have the same symptoms. Been to the Doctor and the Dermatologist. Doctor can't find any problem, Dermo says it's atopic dermatitis. I just think the Dermo is guessing because he doesn't know either. I have had all the blood workups and tests, they can't find anything wrong. Doc said "you sure you're not imagining this?"
Anyway here's what I have found, maybe it can help some of you too. I have researched something called candida... it has a lot of symptoms but the main one is food allergies. This has worked (somewhat for me)... I got some herbal pills at wholefoods called "candidase"

there's many products that say they do the same thing on the shelf, I picked this one because it was the cheapest.
My research on candida showed that a diet is important... but it's a tough one. No Alcohol, no flour, no sandwiche meats, no yeast, no sugar, no dairy, no vinegar (except apple), no wheat, no coffee... basically salad, vegetables and freshly cooked chicken.
Anyway I started taking the pills, went on the diet, and started running everyday.
It's been a week and one day I had mild symptoms but that was it... mostly it's gone. That said, I spoke to a homeopath, and they said I will need at least a month on the diet before i can return to normal eating. Hope this helps.

Reply to HKSTORY and matt238. My symptoms are similar - stinging and prickly irritation of the scrotum that occurs when sitting and gets worse when the area gets sweaty. There is no itching or feeling that I must scratch and no pain when urinating (so cannot see that it is related to a yeast infection).
I have tried many creams and antibiotics for bacteria, fungal and viral infections but none seemed to work. I was also prescribed amitriptyline (an neurological and antidepressant drug) but did not take it for fear of its side effects.
My condition does not occur when I am standing, walking or lying down or sitting on the toilet (O ring seat) but only occurs when sitting normally, which seems to me to indicate that it is a positional problem possibly caused by the squashing or pinching of nerves perhaps. Anyway, I am now trying to get my doctor to refer me to a neurologist and possibly a physical therapist who have experience of problems in this area. Will keep you posted. Please let us know if you have any luck.
