Stress- palpitations occur becaus the heart rate is too high. Which causes a skipped beat (heart resetting itself) caffeine makes this worse.
Acid reflux- maybe you don't feel acid seeping into your lungs but it could be. Chocolate and caffeine makes reflux it worse. (which causes a cough)
Asthma/bronchitis- lack of oxygen can cause this. This can be worstened by lying down.

I am glad to see these posts. I too have had the same. Heart feels like it's fluttering and then I feel the need to cough, which makes the fluttering stop. It may do it agian, but doesn't seem to happen more than three times in a row. I am currently (2012) age 45 and have had this for years, usually at rest. Had a heart monitor on for a couple of weeks, but it never showed anything. Dr. did heart test after I had a gall bladder attack, but when I went in for gall bladder surgery, they re-did the test and said the first was a false positive. It happened out of nowhere. Went to Cardiologist after that, and he said there was no problem. Years ago I had thyroid test that showed nothing out of the ordinary, but just as we now know the earth is not flat, I believe so too will thyroid test improve and show how much our thyroid actually is affected. I have been hypoglycemic since a child and now diabetic as of last year, so maybe the meds I'm taking will improve the problems. Much like Keith, I felt my heart stop while sitting and watching TV as a child, my breath stopped and then heart started back up again.
