Heavy bleeding as soon as the intercourse had began

i began to bleed very heavy as soon as intercourse had began and was in extreme pain, it continued with large clots and a steady stream of blood for about 7 hours. I went to the hospital they had no answers for me. i have been spotting ever since, its been 5 months now. i did go for an ultrasound no results yet. any thoughts??
