I've had this issue since I was 27 and I'm now 42. I've been searching for something online that can help me and until now have found nothing. I've been through many of the procedures mentioned. I do agree it must have something to do with pelvic, sacral, pinching. But through some experimentation have found a way to have almost normal orgasms again.
this is going to sound weird but if I masturbate on my back in a bow position, with pillows under my pelvis and shoulders no yead, it somehow makes the orgasm almost normal. I can feel the engine starting like in an old car because it's been so long since those areas were stimulated but, somehow this position somehow relaxes rage pelvis or something.
If anybody finds a similar position brings relief please let me know. And thanks for the posts!

I have the blocked sensation during ejaculation - used to be 10/10 sensation of release but since this issue 1/10 if lucky - ongoing for the last 20 years, i am now 43. This began after a single night of rough sex and holding off ejaculation to the extreme.
Over the years i have spoken to a few dr's whom have been clueless / said that this cannot happen. This has made it very hard to follow up the cause. Anyway i have again raised this with my current dr and got referrred to a urologist.
As a result i had a cystoscopy (camera up the penis, f$$$ painful) yesterday via the urologist to check for a possible stricture - however anatomy is normal I was told, hummm, in some ways i was hoping stricture was the cause then at least i'd have a diagnosis and some chance of known treatment.
Anyway the only thing the urologist said was the next possibility was that the pelvis muscle area where uretha passes through it was overly sensitive and tight in my case. I was told that tightness in this area may (this was his next best guess btw because he did say b4 the procedure that if it wasnt a stricture then he would have no idea...) be the cause and that i should seek myofascial release of the pelvic floor from a Physio.
I have been searching online since but so far what i have discovered is a) very few physios do this ( because the method is via the rectum(no kidding)) and b) the ones that do are for women who have non bacterial cystitis or incontinence.
I'll keep searching for a physio who does this as i am willing to try and somehow fix this however unpleasant the treatment, but i am not holding out much hope of finding one here in nsw.
Anyone else tried this at all??
Nb. Interesting others here have mentioned possibly pelvis related given what my urologist said. i was also thinking some kind of internal hernia perhaps seeing how it occured, but if the specialists dont know then its all guess work.

your story sounds very similar to mine, i live in nsw too although I'm only 31 but it seems my problem started the same age as you around 23-ish. I had different stuff done, ultrasound, urine tests, blood tests and also cystoscopy, but nothing was found out of the normal.
I definitely feel it is pelvis related after reading through this thread. In the 10 years I've had this problem I haven't found anything that has relieved the pain, although I wouldn't say my ejaculations are a 1/10 more like a 5/10 only half as good as they used to be. It is still very depressing that no specialist can seem to find anything. Maybe some things just can't be fixed?
