Once when I was masterbating, I pulled back my foreskin all the way (so the head was revealed) and my head started to swell up. Is that normal? Is that how it's supposed to be done?

Many penis heads, when not being choked by having the foreskin down around the tip,will swell larger. If the foreskin is retracted tightly, the rim of the head will often flare out as well. Both of these are common.
But lets be sure you don't have something different which is a problem. Are you sure the penis isn't swelling because retracting of your foreskin is cutting off blood circulation behind the head? That is also common, but is a problem that should be corrected. It is known as paraphimosis. The opening must be stretched, or the foreskin circumcised.
