I have the same problem. The reason why urine comes out instead of cum is because you're touching a 'cushion' that connects to the bladder inside you. This sends messages to the bladder causing you to urinate instead.

I would recommend starting by stroking and gently squeezing your nipples, if your titties are big then you may be able to suck your own nipples, that feels amazing! If not keep using one hand to stimulate the nipple while moving the other down.

i'm also 16 and this happens to me too. i read that since you're clit is next to the bladder and when you masturbate it puts stress on you're bladder, so you pee. it should eventually stop. it said to pee first to try and prevent it. i'm starting to pee less and less. good luck fulfilling your needs.
