So here I set, ten years later, at 4am on the couch wondering what the hell is wrong with me. None of my guy friends have this problem.
The pain is a very sharp, stab-like pain accompanied by a burning sensation. There is an urge to pee still, but it just dribbles and when I clinch, the pain is magnified for that second. When touched, only the tip responds with a feeling of sharpness. The shaft has no pain.
Around a year ago, I realized that I hold my pee too long and had a very good hunch that was the source of my pain: if I held too long, there would be pain when I finally peed. Then I figured out if I don't completely finish peeing when I held it for too long then there is no pain.
This past week, the irritation has come quite often. The only thing I can think is because I've been lowering my intake of caffeine. Nothing else has changed. Also, the pain is there even if I don't finish peeing like I mentioned above.
So, my possible instigators: holding too long or diet (caffeine).

Try to keep these muscles as relaxed as possible during sexual arousal, and see if the pain still persists afterwards. Further more, check to see how your general flexibility is in your lower back and hips. Chronically tight muscles could be both indicative and causative of pelvic floor dysfunction - when these muscles are tensing (which they should) they are either over tensing or maintaining tension for an inappropriately long time post arousal, causing nerve aggravation.
Why in the tip of the penis if all of these problems are occurring in the pelvis? - the is called referred pain. The nerve paths to neighbouring regions of the body are shared, this is why shooting pains down the left arm occur during a heart attack, when there is actually only pain generation at the site of the heart. You might find that the pain is biased to one side more than the other (left vs right). This could represent asymmetries in the pelvic and hip musculature. If this is all ringing true, then there is hope for recovery, but the bad news is that rehabilitation and retraining of the pelvic floor takes time and dedication.
There are some books devoted exclusively to this topic: "A headache in the pelvis" by David Wise. There are a few documented cases of exclusive pain in the tip of the penis, and a protocol for overcoming these never issues.
I hope this helps. I have been suffering from this problem for more than 6 years, and it is only in the last 6 months that I have found the strength of will to dedicate myself to correcting the problem..... I hope to be able to post at some point in the future that I have made some serious improvements. Good luck to anyone else trying to seek resolution, I know how psychologically tough it can be. Yeah, this is quite common problem and I have went through the same as others. Urologist , as soon as they detect a problem with your penis it could only mean two things to them... STD or Prostitius when the urine and swab test come back clean. Its like egg on their face at that moment so usually they recant their statement.
