That piece of skin at the bottom of the glans is called the frenulum. Almost every male is born with a frenulum. It is normal and it is supposed to be there. The frenulum has a high level of sensation because of its high innervation and serves to limit the rearward travel of the foreskin. The stretching of the frenulum during sexual intercourse helps to trigger ejaculation.
If your foreskin bunches up behind the head when you retract you most likely have a short frenulum (frenulum breve). Test it by pulling back on the foreskin. If the head of your penis bends down when you pull back your foreskin, you have frenulum breve.
You can stretch the frenulum manually just like you do your foreskin. That is the best course of action if the frenulum is too short to permit your foreskin to retract. Search the Internet for:

There also is a minor operation to relieve the frenulum, called frenuloplasty and another called frenectomy. One of these might be necessary, if the stretching does not work. Urologists do this.

The frenulum is located at the very bottom of the head in the center. If you are reporting a connection off to the side, then is is likely that you have an area of fusion.
Baby boys are born with the inner layer of the foreskin fused with the surface of the underlying glans penis. This is natural, normal, and protects against infection. Over some years the fusion spontaneously disintegrates and releases the inner foreskin. This usually happens prior to puberty, but not always.
It is not uncommon in adolescence to have the inner layer of the foreskin still fused to the glans penis. It is not a serious problem.
Apparently, your foreskin has not yet released, although it probably is about ready to release due to your age. You can try peeling the foreskin away from the glans penis to see if you can break the remaining fusion. Do it carefully and little by little. It may take you several tries to get it all.
When you tear the foreskin away, the head will be red in color, where the separation occurred, for a while. Put some Vaseline under your foreskin to prevent it from re-adhering to the head.
You may also try "ballooning" your foreskin by pinching the tip shut when you pee and causing the foreskin to fill with pee. That will lift the foreskin away from the head and tear loose the fusion.
If you can’t get it to separate by pulling on it, a urologist can help you. It is called “lysing”. Don’t let anyone sell you a circumcision. It is not necessary and extremely harmful.

It is normal for the glans to be sensitive because your foreskin has been protecting it for your whole life.
The foreskin is there to protect the head of the penis and keep it moist and sensitive. If it hurts, don't touch it. Keep your foreskin forward to protect the head. It will probably get less tender later.
I don't think anything is wrong with your penis. It sounds normal to me. The head is supposed to be sensitive.
There is no reason for you to touch the head so just don't touch it. Pull your foreskin forward to cover the head to protect it and you should be fine.

I had a similar problem to yours when i was around 10 yrs old. With me, the problem was that the foreskin should have broken away from my penis head, but it hadn't. My doctor told me just to clean it gently with normal soap/body wash in the shower, and dab it dry it after urinating. After a few years, the foreskin came loose, but the area it had been covering was sore for a few days and hurt whenever I exposed it to air, let alone touched it. After a few days, though, it felt fine. I found it best if I pulled back the foreskin completely every time I went to urinate or shower, and just to ensure it was completely clean after I was done. If your problem is similar to mine in the few days that mine hurt, you desperately need to thouroughly clean your penis head. Thats the problem, and unless it is clean enough for the sensitive area to be exposed to air, your penis will always feel as sensitive as it does now. I would try to keep it covered by your foreskin for most of the time, but when you shower, pull back your foreskin slowly and carefully, and wash some of the dirt away. And definitly talk to your doctor about it. I'm just going off what I remember , but your doctor will know best.

Sorry you're going through this -- sounds awful.It sounds like balanitis -- inflammation of the penis head and foreskin caused by lack of hygiene. You could have something else going on that's causing difficulty and pain retracting the foreskin -- seeing a doctor is the absolute best advice. Some guys heal with home treatment, but in severe cases, circumcision is called for.
