Starting with background. Father about to turn 40 working out with son home from college. 5 th week into it going 5 days a week were on legs. Im in good shape laying my back feet up on the sled 3rd set i get a pull in my lower back. Struggled a bit that set. Not a sharp pain but tooth ache in my back, but also heft him. Was weird feeling but finished the remaineder of my training for the day. Was a little achey but ok until I sat for the drive home, pain wrapping from my left testicle to my lower back and kidney. All i can say is wow for the pain getting out of the car. My left testicle was in pain not sharp not dull but relentless tooth ache in my lefty. I get throbes in lower back and burn inmhip at times. So hot shower ahhhh relief but Oooooo dont touch it.. no dinner bio freeze lower back and cheek. Sleep with pillow under my knees and wake up stiff but hot shower and off to work little pressure but no biggy. Took evening off felt ok.. next day went to the gym chest daymworked out fine had a small bit of pressure and burn in lefty on decline bench. Stopped and felt fine until... driving home and yeah wow times 10.

i have had the same issue for almost 2 years it started while i was dooing some heavy lifting.i went to a uroligest that said i got tramadic epmiditimites that went away. but i was still in a lot of pain so he sent me for catscans for a hernea not that. i was sent back to work a few months later i was putting a starter on the work truck. and bam it was back so i got a referl to a neuroligist they did a test and said I had minor nerve damage. the pain kept worsning i would be bent over at least 45 degreas. he had me on hydrocodone and as the pain kept growing i took more to keep the pain down never even got close to the max dose then he ct me off and gave me Tramadol sray away from this stuff it will mess your head up so i told workers copm to send me to pain managment. the workers comp adjuster did not beleave me since the pain would be deep in my hip or in mt groin or testecial or all 3 at once prodomantly the right side so she sent me a nurse case manager that in 5 min sitting in the wating room of the pain managment doctors office i had an injured back. low and behold the mri coms back just catching T 12 and L1 bulging disk now it is almost a year later i just had a fusian and discetomy now my right side hurts like a mfer. and the doctor is starting to act like i am faking it i dont know what it is but the pain sucks and a month out and the surgery did nothing for me so far i dont know if it is because it took almost 2 years but any one that ends up on this page if you are in the pain i am in i wouls not wish it on my worst enemy. and i wish you luck finding out a way to fix it

Hopefully this will help some of you as a lot of these were very similar to a pain I felt.
I felt a pain that I couldn't quite pinpoint. It felt like a soreness in the "cord" attached to my left testicle, but it was also on the right side about 10% of the time. After an NP, testicular ultrasound, Urologist, pain management specialist, and sacroiliac joint steroidal injection, I saw a new general practitioner. She suggested physical therapy for some kind of referred pain originating from the pelvic floor. The next day she called me and suggested it may also be a sports hernia(A sports hernia is technically not a hernia, and you would NOT be able to feel with your fingers a bulge. It also happens almost exclusively in very high level male athletes{I'd consider myself in good shape, but I'm not exactly in the NFL}).
I saw a physical therapist that diagnosed me with a hip adductor(groin) strain(different from a groin pull, which I've had and does not feel like this), and gave me exercises to treat it. For my second session of PT, the first therapist was out of town, so his superior saw me. They had discussed my case already and after another significant examination, he said it could be either one, a hip adductor strain or a sports hernia. Either way he tells me, the exercises/treatment is practically the same. I'm starting to feel better already and it's been two weeks of PT. Maybe it's actually getting better, but it's worth looking into, especially a sports hernia. When I read a little bit about that(a.k.a. athletic pubalgia), it read like a biography of the last few months of my life.

Men experiencing this type of pain may want to try abstaining from sex and/or masturbation for a few days and see if the pain lessens (give it at least 72h). I had this type of pain and I was freaking out about it for a while but I recently tried what I've suggested and recently it has gone away completely. I will update this forum if it returns to let you guys know, but for now this simple self treatment has really helped. I'm sure if its just a muscular injury it will quickly heal over time and I can resume my sex life :).

I've had these symptoms for about 10 days now, starting on the right and moved to the left groin area. It seems to migrate from low to high during the day and is definitely worse when I'm sitting. I read a few posts last night that recommended seeing a PT for deep tissue massage on the lower back so this morning I did some intense back stretching and then laid on a stiff tennis ball, rolling along my spine/kidney/lower back area and I've felt much better. I also put a tennis ball between my back and office chair and it's helped a lot. Good luck

I'm 27 and I've had this problem for nearly 4 years. I get a pain in my left groin & testicle due to playing football (soccer) and more specifically sprinting and changing direction.
The first time I got it I thought that it was because I was wearing loose fitting underpants at the time and so I started wearing tight underpants and after the season finished I rested it for a few months and the pain disappeared. Then during pre-season I was doing sprints and I could feel the pain again and it got worse and worse the more I played on it. I then took a year off to focus on my exams and so when I returned I thought I'd take it really easy and build up the intensity of my training sessions slowly. I also did some execises to stengthen my pelvic floor muscles. It took a few months into the season for me to start feeling the pain again and once again it came after I had done an intense sprinting drill.
I have recently gone to the doctors for the third time but this time emphasized the fact that my groin was painful too and didn't focus on the testicle too much. He did an examination and diagnosed me with Gilmore's groin (aka sports hernia, athletic pubalgia). It was painful when he touched the point where my ligament in my groin attached to the bone. I will now have 6 weeks of physiotherapy (which I hear has a 15% sucess rate). This involves doing squats and lunges with weights to strengthen the area (the physio says it's important to hold weights so that you are carrying more than your body weight). If this fails then I would have to have surgery............

I've had this exact problem.....believe it or not, its "referred pain" from a muscle in your obliques (left side). Go see a registered massage therapist that specializes in "trigger points"....he will be able to fix the problem. Of course you should also consult a doctor if you're peeing blood or pain when you ejaculate.

I am a junior doctor and have had a very similar issue to many of you guys. I feel I may be able to offer some advice.
I began in June '12 with a sudden ache in my right testicle, was sat in the car at the time. No preceeding symptoms/infection etc. Tried antibiotics- no help. Had an USS which showed possible thickened epididiymis on R side- had sexual health screen- all negative.
Referred to a urologist who suggested it could be renal/kidney related as i started with some pain in the R loin/abdo as well. CT scan was normal i.e. no stones/renal pathology. Have since seen infectious disease consultant and further urologists, had a cystoscopy that was normal. I am currently taking a months worth of abx in case it is prostate related.
It sounds very much like many of you have a chronic epididymitis or some form of strain/groin hernia. It is worth trying a few weeks of ibuprofen 400mg 3 times per day. Hot baths will help many of you, as they did for me.
In the first instance you should see your doctor and suggest an ultrasound, to rule pathology out. Rarely, testicular cancer can present with aching testicle (although this is usually painless). Acute epididymitis will usually be the result of infection, and your testicle will swell and the pain will be bad. For most of us, pain is more of a dull ache and there is no swelling- making infection less likely.
Supportive underwear rather than boxers can also help, these support the testicles rather than them hanging on the cremaster muscle.
Suggestions
* 2-3wks of ibuprofen/anti-inflammatory (400mg 3xper day)
* Regular hot baths
* Supportive underwear.
From the data i have seen, i wouldn't personally recommend invasive surgery such as epididymectomy/orchiectomy as the results are not great and they compromise fertility. Although i am not an expert in this field.
There is a fairly new procedure called microsurgical denervation of the spermatic cord that is offered in the US. They report results of >80% success at reducing pain by over 50% and many of their patients have complete resolution of pain.
Google microsurgical denervation of the spermatic cord and check it out yourself.
I am currently contemplating whether this is my own next step- it is good in that results are better, and it doesn't affect erections/sensation of the scrotum and does not remove any tissue vital to fertility i.e. epididymis and testicle.

This thread gave me a huge relief and pointed me in the right direction. Scary stuff, but the exercises helped me. I wanted to take the time to give back, for anyone out there in the future who might have a similar issue, by detailing what I felt, and what helped. Bottom line: For me this is mostly a muscular issue, likely based on bad posture. Adductors and Hamstrings too tight. Psoas too tight. Combo antibiotics and stretching / PT = resolution in my case.

Noticed dull ache in right side scrotum. Almost like someone kicked me in the nuts, but 1/100 strength. No lumps. Then after about a week, it went away. Didn't think about it again until 1 month ago.
One month ago (starting July 2nd, from journal:)
Did stair climber on July 2nd (no idea if related) Started feeling again on around the 3rd. Jumped into ocean and felt a tiny jolt in same area on July 5th. Continued through weekend and through 8th. Did lots of stretching on 7th, but no real help. Lot of self-exam, nothing sinister found.
Still there on 10th, but took 2 ibuprofen and all symptoms gone that night. Perhaps inflammation, perhaps infection. Today (11th) symptoms are much less than previous. Maybe going away. Best description is discomfort, not ache or pain.
Went walking last night (11th) and felt a little worse sleeping. Had 2 ibuprofen again, but this didn’t stop it waking me up. Just uncomfortable. Was again discomfort at work this morning while sitting. Worried that the walking was making it worse, and maybe a hernia. Went to Doctor today (12th). Thought small infection, so gave antibiotic, but said if it didn’t work, would need to go for ultrasound to check for small hernia. He couldn’t feel anything though.
Antibiotics (2 days in) working. Far less feeling at night. Occasional feelings like before, but few and far between. Big difference is now the testicle doesn’t feel it much, but I have a pain lower. Like ½ inch from my anus. It’s like a nerve or something. Like after sitting on a hard surface for too long. It’s a harder pain, like muscle. Have had this before I think. So, the testicle pain might have been referred from bad posture? Or maybe there was some infection. It’s not ‘gone’, but it is better. Tomorrow at work first day will tell. Also – have not walked much, and this seemed to be a big cause / exacerbater
5 days into Antibiotics. Had a bad day yesterday. The night before during sleep I felt it a little, after swimming that day. Got up the next day convinced I had a hernia, so started poking around and inside the cavern… and of course this made it ache there all day. Not bad, but enough. Walked that night, and it did NOT get worse like before, so this was good. However, went to bed and woke up feeling it there (slightly above scrotum). Read on Internet all day again and found many people had this problem from tight back / bad posture. Did stretches, and it does seem better, especially adductor stretch. Praying this is a posture / stretching thing. Will work on this. Potential is there for psychosomatic for sure. Very nervous.
Wife started doing deep lower back massage on Thursday 18th. This was really tight! Pain started to migrate from the scrotum up toward a point just below my hip bone. At first I thought hernia again, but the pain really seems to center now at the point below my hip. Scrotum issue seems diminished, and stretching seems to have helped the adductor ‘issue’. Now going to chiropractor today (July 24th) to see about the spot below the hip. Tomorrow to massage therapist.
Both Chiro and Massage Therapist felt this was a muscular issue and their treatment of such was starting to help. MT worked on back only, Chrio on back and releasing the Psoas. But, always felt worse for a few days after massage. Normal. Needed piece of mind, so got ultrasound. No hernia, but slightly raised fluid in tubes around testicle. Maybe need more antibiotic, but will ask for physical therapy first.
