28m, circumcised, do pretty good at keeping myself clean.

So about a week ago I had my girlfriend over and we got in the mood, went to the bedroom, as soon as I pushed it inside of her(used condom and water based lube, as always) I felt a pain, so I go check it out and there's a small line around part of my head that was broken skin. I put neosporin on it, kept it clean, redness and everything is gone, I have inspected it and part of my foreskin is actually attached to the underside of my head, right at the ridge that goes around. No idea how this happened as we have sex usually every weekend and all was fine before this. 

Never had any issues with anything, penis related. Especially nothing like this. 

What can I do to remedy this? What has caused it? I'd rather not go to the doctor if I can avoid it, but I'm not sure who I would go to?

Look up "frenulum."

I believe that is what you are seeing, circumcised or not.  Yes, it often goes to the corona.

You just may have torn it slightly during sex - it does happen. 

Let it heal.  It is unlikely anything is wrong.

It is certainly not the frenulum. I guess I didn't explain it well enough. It is to the left of the frenulum, and it is a 1/4-1/2in section of foreskin that has attached just under the ridge that goes around. It is certainly not supposed to be attached, nor has it ever been attached. It happened just recently, and it tore when I inserted inside of her because it's not supposed to be attached. Would a picture help?
